术前动脉化疗对进展期胃癌细胞凋亡和增殖的影响

摘　　要：目的评价术前动脉化疗对进展期胃癌细胞凋亡和增殖的影响。方法应用原位末端标记法(TUNEL)及免疫组化技术对56例进展期胃癌肿瘤组织和局部淋巴结转移癌组织标本进行凋亡和细胞增殖检测;其中38例行术前动脉化疗,另18例胃癌组织作对照。结果对照组胃癌组织细胞凋亡率为4.4%±1.5%,局部淋巴结转移癌细胞调亡率为6.4%±3.5%;而术前行动脉化疗组细胞凋亡率为10.6%±4.5%,局部淋巴结转移癌细胞凋亡率16.6%±5.5%,两者差异均有统计学意义(P<0.05)。增殖细胞核抗原(PCNA)的阳性率对照组为77.8%(14/18),而术前化疗组为47.4%(18/38)。结论术前动脉灌注化疗可促进进展期胃癌组织及局部淋巴结转移癌细胞的凋亡并抑制其增殖,可作为进展期胃癌术前辅助化疗重要方法之一。Objective To evaluate the apoptosis and proliferative response of preoperative intraarterial chemotherapy for advanced gastric carcinoma. Methods 56 patients with advanced gastric cancer and local metastatic lymph nodes were determined whether to have apoptosis and proliferation by TUNEL and immunoehemical technology, including 38 cases treated by preoperative intraarterial chemotherapy and 18 cases used as controls. Results The rates of apoptosis of gastric cancer and local metastatic lymph nodes in control group and preoperatively treated group were 4.4% ± 1.5% and 6.4% ± 3.5%; 10.6% ± 4.5% and 16.6% ± 5.5%, respectively. The difference between two groups had statistic significance（P 〈 0.05）. The positive rates of PCNA for the control group and preoperative treated group were 77.8% （14/18）and 47.4% （18/38）, respectively. Conclusion Preoperative intraarterial chemotherapy can promote apoptosis of advanced gastric carcinoma and local metastatic lymph nodes and restrain their poliferation, serving as an important useful adjunct for advanced gastric cancers. （J Intervent Radiol, 2007, 16： 353-356）
