阿尔茨海默病患者Aβ_(42)抗体测定的临床意义

摘　　要：目的探讨 ELISA 法检测β淀粉样蛋白(Aβ)_(1-42)自身抗体在诊断阿尔茨海默病(AD)中的临床应用价值。方法用 Aβ_(1-42)蛋白片段包被 PVC 板,使 AD 患者血清与小鼠抗 Aβ_(1-42)IgG 形成竞争,以辣根过氧化物酶标记的羊抗鼠 IgG 作为二抗体,通过竞争 ELISA 法测定和比较健康人、高血压性脑栓塞患者及不同年龄的 AD 患者血清 Aβ_(1-42)抗体水平。结果 ELISA 检测 Aβ_(1-42)自身抗体法的灵敏度约为1 ng/ml,回收率介于96.5%～104.7%之间;对 Aβ_(1-42)自身抗体被吸附去除后的人血清以及马血清的 Aβ_(1-42)抗体水平<1 ng/ml。37例 AD 患者血清Aβ_(1-42)自身抗体水平为(5.1±1.9)ng/ml,明显低于同龄健康人的(12.6±3.3)ng/ml和高血压性脑栓塞患者的(12.5±3.3)ng/ml,差异均有统计学意义(双样本成组等方差 t 值分别为:6.237、7.973、6.224,P 均<0.01)。结论 AD 患者可能清除 Aβ能力不足,所以血清 Aβ_(1-42)自身抗体水平明显偏低。Objective Clinical significance of using ELISA to determine β-amyloid （Aβ）1-42 antibody levels in the sera of patients with Alzheimer＇ s disease （AD）. Methods 96 wells PVC plate was coated with Aβ1-42 peptide. Serum of AD patient was competing with mouse Aβ1-42 monoclonal antibody in this assay. The second antibody was horseradish peroxidase （HRP） conjugated goat anti-mouse IgG. Serum Aβ1-42 antibody levels were determined by ELISA. Results The sensitivity of this assay was about 1 ng/ml. The recovery rate of this test was between 96. 5% and 104.7%. The residual Aβ1-42 antibody levels in human serum or horse serum after Aβ1-42 antibody was removed by absorption were less than 1 ng/ml. Serum Aβ1-42 antibody levels in 37 AD patients [ （5.1± 1.9） ng/ml] were remarkably lower than those in normal people [（12.6±3.3） ng/ml, P〈0.01] or in brain thrembus patients [（12.5±3.3） ng/ml,t = 6. 237,7.973, and 6. 224, respectively, P 〈 0. 01 ]. Conclusions Serum β-amyloid （Aβ）1-42 antibody levels in AD patients are significantly lower than those in normal people. Decreased serum Aβ1-42 antibody levels in AD patients indicate that their clearance ability of Aβ is not enough.
