摘　　要：目的探讨检测非小细胞肺癌(NSCLC)患者血液中TA,CEA mRNA,CK19 mRNA的临床意义,评价三者诊断NSCLC血液转移的应用价值.方法NSCLC组74例,非肿瘤性呼吸系统疾病患者(疾病对照组)51例和健康人(健康对照组)36例,应用端粒末端重复序列扩增-杂交-酶联免疫检测技术检测TA,逆转录-套式-聚合酶链反应技术检测CEA mRNA和CK19 mRNA.结果NSCLC组血液TA,CEA mRNA和CK19 mRNA阳性率高于两个对照组(P<0.01),且TA和CEA mRNA的特异性和敏感性也高于CK19 mRNA,但不同类型NSCLC检测3个指标结果的差异无统计学意义(P>0.05).结论检测血液TA,CEA mRNA和CK19 mRNA有助于发现NSCLC细胞血液转移,且TA和CEA mRNA的价值高于CK19 mRNA,但此3个指标可能无助于鉴别NSCLC的类型.AIM： To explore the clinical implications of detec- tions of telomerase activity （TA）, carcinoembryonic antigen （CEA） mRNA and carcinoembryonic antigen CK19 mRNA in the diagnosis of blood matastasis of non-small cell lung cancer （NSCLC）. METHODS： Blood samples from 74 cases of NSCLC （NSCLC group）, 51 cases of benign pulmonary disease （disease control group）, 36 healthy persons （ healthy control group） were detected for CEA mRNA and CK19 mRNA by reverse transcriptase-nested primers-polymerase chain reaction （RT-NP- PCR） and for TA by telomeric repeat amplification protocol-hybridism-enzyme-linked immunosorbent assay （ RTAP-Hyb- ELISA）. RESULTS： The positive rates of CK19mRNA, CEA mRNA and TA in blood of the NSCLC group were far higher than those of the other 2 groups （ P 〈 0.01 ）. The specificity and sensitivity of CEA mRNA and TA were higher than those of CK19 mRNA. There were no differences in the 3 indexes among the different types of NSCLC （ P 〉 0.05 ）. CONCLUSION ： The detectionsof TA, CEA mRNA and CK19 mRNA in blood are useful for the diagnosis of blood matastasis of NSCLC. TA and CEA mRNA are more sensitive than CK19 mRNA, but the 3 markers may not be helpful to distinguish the sort of NSCLC.
