摘  要:目的建立孕兔失血性休克动物模型,为研究孕产妇休克的变化规律及寻找有效的救治方法提供基础。方法参照国内外非妊娠动物失血性休克模型制作方法,选取孕兔为研究对象,随机分为假休克组(P组)、休克未治疗组(P0组)和休克治疗组(PS组),每组10只。通过观察失血性休克时血流动力学和肾脏病理,首次建立孕兔失血性休克模型。结果P组整个实验过程中血流动力学均保持稳定,说明各种手术操作对其血流动力学影响不大。P0组整个试验过程表现为典型休克状态及转归,失血68ml(38%)时出现休克症状,此时MAP为(40.0±3.5)mmHg、呼吸频率(115.6±9.12)次/min、心率(361.7±37.2)次/min、CVP降至(-0.12±0.36)cmH2O,实验第80min出现死亡,第180min全部死亡。PS组:急性失血后即实验第30min呈现典型休克状态,经过输血、输液治疗,血压回升、呼吸心跳平稳,实验第180min全部恢复正常。肾脏病理改变:P组肾脏无明显病理改变;PS组肾脏大体结构正常,肾小球内可见少量炎性细胞存在;P0组中呈现典型休克病理改变,肾小管可见明显玻璃样变性、坏死,肾小球和肾小管周围可见大量炎性细胞浸润。结论通过颈动脉放血、剪开孕囊出血等方法复制了仿真性好、可塑性强的孕兔失血性休克模型,为进一步研究孕产妇休克的变化规律,探讨有效救治方法奠定了基础。Objective To study the hemodynamic changes in maternal hemorrhagic shock and find effective treatment with the newly established hemorrhagic shock model of pregnant rabbit. Methods With reference to hemorrhagic shock model of non-pregnant animals, the pregnant rabbits were randomly assigned to sham shock(Group P), shock without treatment (Group P0), shock and resuscitation groups(Group PS, 10 rabbits for each), and the hemodynamic changes and kidney pathology were studied. Results The hemodynamic parameters in Group P remained stable throughout the experiment, which meant no significant influence of surgical procedure. Rabbits in Group P0 were in shock when 68 ml of blood were lost (equal to 38% of their total blood volume) with the mean arte- rial pressure(MAP) and central venous pressure(CVP) decreased to (40.0±3.5)mm Hg and (-0.12 ±0.36)cmH20,and the respirative rate and heart rate increased to (115.6±9.12) times per minute and (361.7±37.2) times per minute, respectively. One animal died at 80 rain after hemorrhage and no animal survived at 180 min. Aninmals in Group PS were in shock at 30 min after hemorrhage. After resuscitation, all animals were recovered at 180min. No obvious pathologic changes was seen in those of Group P, while those in Group PS showed normal gross structure in kidney except for a few inflammatory cells infiltrated in the glomeruli. In Group P0, massive inflammatory cells infiltrated in the glomeruli and nephric tubules and hyaline degeneration and necrosis were seen in the nephric tubules. Conclusions Through cutting a medium blood vessel of gestational sac and withdrawing blood via carotid artery, the hemorrhagic shock model of pregnant rabbit was established to mimick the clinic situation and lay a foundation for further study of hemodynamic changes in maternal hemorrhagic shock.
