摘 要:目的观察组织工程角膜等效物(tissue-engineered corneal equivalent,TECE)大鼠穿透性角膜移植后大体及组织病理学特点。方法脱细胞猪角膜基质(corneal acellu-lar matrix,CACM)接种7dSD大鼠原代角膜上皮细胞与基质细胞,气液面培养4~6d至材料透明后作为TECE组植片,单纯CACM作为对照组植片,施行穿透性角膜移植术。术后观察角膜新生血管、植片混浊度、角膜细胞修复情况,2周、4周、8周、16周取材进行HE检查、20周取材进行透射电镜检查。结果TECE组术后10d角膜荧光素钠染色(-),对照组术后14d角膜荧光素钠染色(-);TECE组新生血管出现时间(5.00±1.22)d晚于对照组(3.80±0.84)d,差异有显著统计学意义(P<0.01),2周时TECE组新生血管面积(15.47±7.01)mm2少于对照组(62.75±29.22)mm2,差异有显著统计学意义(P<0.01)。拆线后2组角膜新生血管均大部分消退,植片逐渐透明;20周透射电镜检查提示2组均有新胶原形成,TECE组有神经纤维长入。结论以CACM为支架构建的组织工程角膜等效物可以抑制角膜新生血管形成,透明度高。Objective To observe the pathologic characteristic after penetrating keratoplasty (PKP) with tissue-engineered corneal equivalent(TECE) in rat. Methods The primary corneal epithelial cells and matrix cells were inoculated with porcine corneal acellular matrix ( CACM), then cultured between 4 days and 6 days to reach the transparence, which were set as TECE group, and the simple CACM as control group, all were treated with PKP. The corneal neo- vascxdarization, opacity of grafts and autologous corneal cells recovery were observed. Buttons were excised at 2 weeks, 4 weeks, 8 weeks, 16 weeks,20 weeks after surgery, hematoxylin and eosin staining analysis and transmission electron microscopy evaluation of the excised corneal buttons was performed. Resuits The corneal fluorescite staining was negative in TECE group at 10 days and control group at 14 days. The time of corneal neovascularization in TECE group ( 5. 00 days ± 1.22 days) was shorter than that in control group ( 3.80 days ± 0.84 days) ,there was significant difference( P 〈 0.01 ). The area of corneal new vessels in TECE group( 15.47 mm^2 ± 7.01 mm^2) was lower than that in control group(62.75 mm^2 ± 29.22 mm^2 ), there was significant difference ( P 〈 0.01 ). The corneal new vessels disappeared mostly after dermal sutured out,and the graft became transparent gradually. The transmission electron microscopy showed that new collagen formed in two groups at 20 weeks, the nerve fiber graw into the graft in TECE group. Conciusion TECE can inhibit the corneal neovascularization, has high transparence and supports the growth of nerve fiber.
