检测重症肌无力患者血清Ryanodine受体抗体和干扰素-α抗体的临床意义——附89例报告

摘　　要：目的：探讨检测重症肌无力（myasthenia gravis，MG）患者血清Ryanodine受体抗体（Ryanodine receptor antibody，RyR-Ab）和干扰素-α抗体（interferon-dantibody，IFN-αAb）的临床意义。方法：采用ELISA法检测89例MG患者血清中的RyR—Ab和IFN-αAb的水平，比较MG患者伴有不同胸腺异常情况（胸腺瘤、胸腺增生、胸腺萎缩）或胸腺正常，不同年龄段（以40岁为界）的RyR—Ab和IFN-αAb阳性率。结果：MG伴有胸腺瘤患者中男性和年龄40岁以上者占绝对优势（分别占86％和82％），有别于其他患者；MG伴有胸腺瘤患者的血清RyR-Ab及IFN-αAb的阳性率分别为77％、64％，明显高于MG伴有胸腺增生患者（5％、5％）、MG伴有胸腺萎缩患者（1／6、1／6）、胸腺正常的MG患者（0.2％）（均为P〈0．01）；年龄41-72岁MG患者RyR．Ab的阳性率（32％）、IFN-αAb的阳性率（35％），均高于年龄17—40岁MG患者（13％、8％）（P〈0．01，P〈0．01，P〈0．05）。结论：检测MG患者的血清RyR—Ab和IFN-αAb有助于在MG患者中甄别出伴有胸腺瘤的患者。Objective ： To investigate the clinical significance of the Ryanodine receptor antibody （RyR-Ab） and interferon-α antibody （IFN-αAb） in myasthenia gravis （MG） patients. Methods： RyR-Ab and IFN-αAb were measured by using enzyme-linked immunosorbent assay （ELISA） in 89 MG patients （ MG group） . The positive rates of these antibodies were compared among MG patients with different abnormality of the thymus or with different ages. Results： Male patients and patients over 40 had higher incidence of thymoma-associated MG（86% and 82% ）. The positive rate of RyR-Ab and IFN-αAb were 77% and 64% respectively in thymoma-associated MG group, significantly higher than those of MG with thymus hyperplasia group （5% and 5%, P 〈0.05 ）, MG with thymus atrophy group （1/6 and 1/6） and MG with normal thymus （0 and 2%） （P〈0.01, P〈0.01, P〈0.01） The positive rate of RyR-Ab （32%） and the positive rate of IFN-αAb （35%） in the group of the age of 41 -72 were significantly higher than thoseofthe patients with the age of 17 -40 （13% and 8%） （P〈0.01, P〈0.01, P〈0.05）. Conclusion： Detection of RyR-Ab and IFN-αAb is helpful in discriminating thymoma-associated MG.
