急性白血病患者红细胞免疫粘附功能的研究

摘　　要：目的探讨急性白血病患者红细胞免疫粘附功能的变化。方法应用红细胞酵母菌花结试验测定45例初次确诊为急性白血病患者化疗前及完全缓解后红细胞膜C3b受体花结率（RBC-C3bRR）及红细胞膜免疫复合物花结率（RBC-ICR）。结果健康对照组RBC-C3bRR及RBC-ICR分别为（18．65±2．58）%、（7．45±2．16）%;急性白血病患者组化疗前及完全缓解后RBC-c3bRR分别为（9．73±3．24）%、（15．12±2．86）%,而RBc-ICR则分别为（13．92±2．75）%、（9．26±2．25）%。急性白血病组化疗前及完全缓解后RBC-C3bRR及RBC-ICR与正常人比较,差异均有统计学意义（P＜0．01）;化疗前与完全缓解后RBC-C3bRR、RBC-ICR两两比较,差异亦均有统计学意义（P＜0．01）。急性白血病患者完全缓解后RBC-c3bRR较化疗前明显升高,但尚未达到正常人水平,而RBC-ICR在完全缓解后较化疗前明显降低,但仍高于健康对照组。结论急性白血病患者红细胞免疫粘附功能下降,测定红细胞免疫粘附功能（RBC-C3bRR、RBC-ICR）可作为判断急性白血病疗效、预后的指标之一。Objective To study the change of immune attached function of red cells in acute leukemia patients. Method By using the red-cell yeast-rosette test,forty-five acute leukemia patients before chemotherapy and aftor complete remission（CR） were detected about red-cells membrance C3b receptors rosette rate（RBC-G3bRR） and red-cells membrance circulation immune complexes rosette rate（RBC-ICR）. Results The RBC-C3bRR and RBC- ICR of normal controls were （18.65± 2.58） % , （7.45 ±2.16） % ;The RBC-C3bRR of acute leukemia patients before chemotherapy and after CR were （9.73 ± 3.24） % , （ 15.12 ± 2.86） % , and the RBC-ICR were （ 13.92 ± 2.75） % , （9.26±2.25 ）%. The results of both RBC-C3bRR and RBC-ICR had more difference as compared with those controls（ P 〈0.01 ）. The results of both RBC-C4bRR and RB（LICR before chemotherapy had more difference as compared with those after CR（ P 〈0.01 ）. The level of RBCPC3bRR after CR patiemts were higher than those before chemotherapy, but were lower than controls, the level of RBC-ICR after CR were lower than those before chemotherapy,but were higher than controls. Conclusion The immune attached function of red-cells in acute leukemia patients is lower. The test of the red-cells immune attached function（RBC-C3RR,RBC-ICR） can be taken for a referential index to iudge the clinical effect and evaluate prognosis of acute leukemia patients.
