自然牙牙裂综合征患者牙磨耗的自身对照研究

摘　　要：目的:调查自然牙裂综合征病例中患牙磨耗模式,评价牙磨耗在自然牙裂综合征(cracked tooth syndrome,CTS)组和自身对照(OC)组间有无差别。方法:49例CTS组患牙,以对侧同名牙为对照组,取研究模观察。结果:(1)磨耗指数:CTS组得1分者8个(16.33%),得2、3分者33个(67.35%),得4分者8个(16.33%);对照组得1分者14个(28.57%),得2、3分者29个(59.18%),得4分者6个(12.24%)。2组相比差异无显著性(P>0.05);(2)相对磨耗:上颌前磨牙舌尖和下颌第一磨牙近颊尖CTS组与对照组具有统计学差异(P<0.05);(3)牙折方向:上颌前磨牙和上颌第二磨牙以近远中向裂为主,依次占各自折裂牙样本的91.67%和88.89%,而上、下颌第一磨牙及下颌第二磨牙的牙折裂方向没有表现出明显趋向性。结论:上颌前磨牙舌尖及下颌第一磨牙近颊尖磨耗相对不足与牙折裂密切相关。Objective：To investigate the features of the natural tooth with cracked tooth syndrome （CTS） and its tooth attrition pattern, and to evaluate the tooth attrition index between teeth with CTS and own control ones. Methods：49 teeth in CTS cases from clinic of department of operative dentistry and endodontics, the Fourth Military Medical University during the year of 2004 - 2005 and 49 own control ones were involved in present study. Study casts were obtained and following characters were studied ： （ 1 ） tooth attrition index; （2） relative tooth attrition degree. Results： （ 1 ） Tooth attrition index： in CTS group, 16.33% of the teeth in CTS scored 1, 67.35% scored 2 and 3,16.33% scored 4; and in control group, 28.57% scored 1,59.18% scored 2 and 3,12.24% scored 4. No significant differences were found between the two groups （P 〉 0.05 ）. （2）The relative tooth attrition： significant differences between CTS and control group in upper premolar lingual cusp and lower first molar buccal cusp were found. （ 3 ） Tooth cracked direction ： it was mesialdistal direction in 91.67% maxillary premolar and 88.89% maxillary second molar in CTS but no obvious direction taxis in first molar and mandibular second molar in CTS. Conclusion： There are close correlations between insufficient tooth attrition in maxillary premolar lingual cusp, mandibular first molar mesial buccal cusp and CTS.
