摘 要:目的建立观察蚕丝人工肌腱止点腱骨愈合及腱腱愈合的动物模型,对重建人工腱的转归进行组织学观察和评价。方法在兔胫骨中上1/3处钻孔,实验侧用经化学处理脱胶的蚕丝,一端穿骨道固定,另一端与趾长屈肌腱在轻微张力下编织缝合;对照侧以趾长屈肌腱内侧1/3的近端穿过骨道固定。分别于术后6周,12周,24周处死取标本,行HE、甲苯胺蓝及I型和Ⅲ型胶原免疫组化染色,观察人工肌腱的腱骨愈合及腱腱愈合过程,与对照组相比评价愈合速度。结果人工腱通过胶原与骨道连接,随着时间延长,连接越广泛,并逐渐出现软骨化及骨化,有形成正常腱骨止点的趋势,但较对照组慢。结论本动物模型稳定可靠,可以作为评估人工腱腱骨愈合及腱腱愈合组织学情况的方法。Objective To observe the healing of tendon to bone and tendon to tendon of silk artificial tendon insertion and evaluate the histological results and biological remodeling of artificial tendon after implantation. Methods New Zealand rabbits were treated by drilling in between the upper and middle third of tibia. At the experiment side, one extremity of silk fibers processed chemically by extraction of sericin was fixed after threaded through the tibial tunnel, while the other extremity was taken and woven to the flexor digitorum longus muscle tendon; at the control side, the proximal end of one third of flexor digitorum longns muscle tendon was fixed after threaded through the tibial tunnel. The animals were killed to get specimens in 6, 12,24 weeks after operation. Standard HE stain, toluidine blue stain and immunohistochemical analysis of type Ⅰ and Ⅲ collagens monoclonal antibody were processed for the observation of the healing of tendon to bone and tendon to tendon of the silk artificial tendon insertion. The healing speed was also evaluated by comparison between the two sides. Results The connection of artificial tendon to bone tunnel via collagens was more and more widespread with the gradual emergence of cartilaginification and ossification in fibrous tissue, along with time going on. There was a trend of normal tendon insertion formation, although it was slower than the control side which was tendon autograft. Condlusion This animal models which is stable and credible, may become a method of evaluating histologically the healing of tendon to bone and tendon to tendon of silk artificial tendon insertion.
