摘 要:目的:探讨中国汉族人群细胞色素P4501A1(cytochrome P450 1A1,CYP1A1)基因型及常见危险因素与胃癌易患性的关系,为胃癌高危人群的确定和胃癌的一级预防提供分子生物学手段。方法:所有研究对象经胃镜和病理检查,分为6组:胃癌组(GC)102例、慢性萎缩性胃炎组(CAG)110例、慢性浅表性胃炎组(CSG)103例、胃溃疡组(GU)62例、十二指肠溃疡组(DU)62例及正常对照组62例。采用统一的调查表对每个研究对象进行调查。采用序列特异性引物的聚合酶链反应(PCR-SSP)法检测CYP1A1基因型测定组的基因型频率,酶联免疫吸附法(ELISA)测定幽门螺杆菌(helicobacter pylori,Hp)感染,病例对照研究法行流行病学分析,分析采用2×K联列表法和非条件Logistic回归方法,分析指标为OR值。结果:单因素分析显示,与胃癌有关联的危险因素为年龄、性别、文化程度、职业、Hp感染、吸烟、CYP1A1G/G基因型,其中大蒜为保护性因素;多因素分析显示,与胃癌有关联的危险因素分别为Hp感染、CYP1A1G/G基因型,大蒜仍为保护性因素。Hp感染与CYP1A1 G/G基因型对胃癌的发生存在协同作用;吸烟与CYP1A1 G/G基因型存在协同作用。结论:CYP1A1G/G基因型的个体发生胃癌的危险性增加,为胃癌易患性的标志。检测这种基因型并且研究这种基因型与常见易患因素的关系能够对胃癌高危人群的确定及胃癌的早期发现和一级预防提供有益的线索。Objective: To explore the association of cytochrome P450 1A1(CYP1A1 ) gene polymorphism and frequent risk factors of gastric cancer in Chinese Han people. Methods: One hundred and two patients with gastric cancer(GC), one hundred and ten with chronic atrophic gastritis(CAG), one hundred and three with chronic superficial gastritis(CSG), sixty-two with gastric ulcer(GC), sixty-two with duodenal ulcer (DU) and sixty-two controls were enrolled in this study. Data on life-style had been routinely collected by means of common questionnaire sheets. CYP1A1 genotypes were detected by the polymerase chain reaction of sequence-specific primers (PCR-SSP). Hp infection was detected by an enzyme-linked immunosorbent assay (ELISA). The approach of case control was used in the analyses of risk factors. Results: Risks of gastric cancer was based on age, sex, education, occupation, helicobacter pylori infection, tobacco smoke, and CYP1A1 G/G genotype. Garlic had a protective effect on gastric cancer. Multivariate analyses indicated that risks of gastric cancer: helicobacter pylori infection, CYP1A1 G/G genotype, and garlic had a protective response. There was synergistic action between helicobacter pylori infection and CYP1A1 G/G genotype, tobacco smoke and CYPIA1 G/G genotype in the development of gastric cancer. Conclusion: An individual with a CYPIA1 G/G genotype is at risk of gastric cancer. This genotype may be considered as a marker related to the susceptibility to gastric cancer. It may provide a clue to define the risk of gastric cancer and lead to diagnosis and treatment at an early stage.
