高渗高胶液体对兔肺挫伤合并失血性休克的疗效观察

摘　　要：目的:研究高渗高胶液体(高渗氯化钠羟乙基淀粉40溶液,HH40)对肺挫伤合并失血性休克兔进行复苏的效果及其可能的机制。方法:新西兰白兔16只随机分为2组:HH40组和乳酸盐林格液(LRS)组。制作肺挫伤合并失血性休克模型,休克60 m in后分别用上述两组液体进行复苏。在休克前,复苏前,复苏后5,30和60 m in分别记录平均动脉压(MAP)的数值,并在休克6 h后将动物处死,取右肺检测其湿干比(W/D),测定肺组织中髓过氧化物酶(MPO)活性、细胞间黏附分子-1(ICAM-1)的表达,并观察肺组织病理结构的变化。结果:血压的恢复HH40组明显优于LRS组,HH40组的W/D和MPO明显低于LRS组(P<0.01),其肺组织中ICAM-1的表达也明显低于LRS组(P<0.01),肺组织病理变化较LRS组轻。结论:HH40注射液对肺损伤合并失血性休克有较好的治疗作用。Objective. To investigate the effects of hypertonic hyperoncoic solution （ hypertonic sodium chloride hydr0xylethyl starch 40, HH40） in pulmonary contusion and hemorrhagic shock rabbits. Methods： Sixteen rabbits were randomly divided into 2 groups ： HH40 group and lactated Ringer＇s solution group （ LRS group）. The animals were created the pulmonary contusion and hemorrhagic shock models. Femoral artery was cannulated for BP monitoring and femoral vein was cannulated for fluid infusion. MAP was maintained at 40 mmHg for 60 min. Then the animals in HH40 group received HH40 5 ml/kg, and those in LRS group 5 ml/kg LRS. MAP was measured before shock （baseline） ,during shock before resuscitation, and 10,30,60 min after fluid resuscitation. The animals were killed six hours later, the right lung tissue wet-to-dry （W/D） ratio and myeloperoxidase （MPO） activity were obtained, the protein expression of intercellular adhesion molecule-1 （ ICAM-1 ） were also detected. Results ： The resume of the blood pressure in HH40 group better than LRS group. The lung tissue W/D, MPO, and the protein expression of ICAM-1 were decreased evidently in HH40 group （P 〈0.01 ）. The morphologic and uhrastructual damages in HH40 group were milder than in LRS group. Conclusion： HH40 injection was effective on pulmonary contusion and hemorrhagic shock rabbits.
