摘　　要：目的探讨经阴道彩色多普勒超声在评价恶性滋养细胞肿瘤血流动力学变化的临床应用价值。方法对31例滋养细胞疾病（葡萄胎16例，恶性滋养细胞肿瘤15例，其中侵蚀性葡萄胎12例和绒癌3例）的子宫的血流动力学状态进行评价。选30例（15例早孕妇女及15例生育期妇女）正常子宫作为对照组。同时检测血清绒毛膜促性腺激素（β-HCG）。结果恶性滋养细胞肿瘤的子宫动脉增粗、扩张、分支增多，具有快速向上陡直的收缩期峰值及向下斜率较高的舒张期血流速度，对照组子宫动脉呈中高速收缩期峰值流速，两者血流参数有显著的差异（P〈0．01）。葡萄胎与正常对照组的宫壁血流，阻力指数（RI）〉0．50，恶性滋养细胞肿瘤宫壁血流异常丰富，RI〈0．40，其血流RI值与血β—HOG下降程度呈负相关（P〈0．01）。结论彩色多普勒显示的异常血流图像和低阻力是诊断滋养细胞肿瘤子宫病变的重要指标。经阴道彩色多普勒超声是评估恶性滋养层细胞肿瘤子宫血流动力学改变的首选方法，对恶性滋养层细胞肿瘤诊断与治疗有十分重要的价值。Objective To evaluate the diagnostic value of color Doppler Transvaginal Scan （TVS） in malignantl trophoblastic tumor. Methods Uteri of 31 patients were observed. They include 16 cases of hydatidiform mole and 15 cases of malignant trophoblastic tumor. The resistance indexes of their uterine muscular layer were measured, and serum β -human chorionic gonadotropin （β-HCG） levels were examined in the meantime. The results were compared with those of normal type. Results It showed that RI of the artery in muscle wall was above 0.50 in hydatidifom mole cases and normal type, while it was below 0.4 in malignant trophoblastic tumor cases. Conclusion Abnormal Color flow image and lower resistance index are the important features for uterine lesions of malignant trophoblastic tumor, color Doppler Transvaginal Scan was a first method evaluate the hemodynamic change in malignant trophoblastic tumor ,Color Doppler TVS has import value in the diagnosis of malignant trophoblastic tumor and its treatment.
