肝细胞癌p53、bcl-2蛋白及微血管密度的检测及意义  

p53 and bcl-2 Expression in Hepatocellular Carcinoma and Their Relationship with Angiogenesis

摘　　要：目的探讨肝细胞癌(HCC)组织中p53、bcl-2蛋白的表达及其与HCC血管形成及临床病理的关系。方法应用免疫组织化学SP法,对石蜡包埋的50例HCC癌组织和10例正常肝组织(对照组)标本进行检测。结果(1)p53蛋白在HCC癌组织(28/50,56%)的表达显著高于癌旁肝组织和对照组(均为0,P<0.01),其中癌旁有肝硬化组(25/3,69.4%)显著高于癌旁无肝硬化组(3/14,21.4%,P<0.01)。bcl-2在癌旁肝组织(62%)的表达高于HCC癌组织(24%,P<0.01)。微血管密度(MVD)在癌组织中高于癌旁及正常肝组织。(2)在HCC癌组织p53蛋白表达与HCC低分化及门静脉癌栓的形成呈正相关;MVD与HCC门静脉癌栓的形成呈正相关,且其值在p53阳性组或bcl-2阳性组均明显高于阴性组。结论p53和bcl-2对促进HCC新生血管形成有一定作用;p53和MVD对判断HCC患者预后有一定意义。Objective To explore the expression of p53, bcl-2 in hepatocellular carcinoma（HCC） and their relationship with microvessel density（MVD） and clinical pathological features.Methods 50 cases with HCC as examined group and 10 cases of normal liver tissues as control group .were examined with SP immunohistochemistry. Results （1）The expression rate of p53 in HCC（28/50, 56%） were significantly higher than that in pericarcinomatous and normal liver tissues（both 0, P〈0.01）, among the positive cases, the rate in the group with pericarcinomatous hepatic cirrhosis（25/36, 69.4 %） was much higher than that in the group with pericarcinomatous non-hepatic cirrhosis（3/14, 21.4%, P〈0.01）. The expression rate of bcl-2 in pericarcinomatous tissues（62 %） was higher than that in HCC（24 %, P〈0.01）. MVD expression was higher in HCC than in pericarcinomatous and normal liver tissues. （2）In HCC, p53 expression has positive correlation with low differentiated and the formation of portal vein embolus of HCC. Also higher MVD was associated with portal vein metastasis. MVD in either p53 or bcl-2 positive group was higher than that in negative group（P〈0. 01, respectively）. Conclusion p53 and bcl-2 may contribute to the angiogenesis of HCC in some extent, p53 and MVD may help to predict the patients＇ prognosis.
