椎-基底动脉供血不足患者血浆ET、CGRP及NSE测定的临床意义

摘　　要：目的:探讨了椎-基底动脉供血不足(VBI)患者血浆内皮素(ET)、降钙素基因相关肽(CGRP)、神经元特异性烯醇化酶(NSE)的动态变化及其临床意义。方法:临床确诊的VBI患者120例,采集发病后不同时间的静脉血,采用放射免疫分析测定血浆ET、CGRP和NSE水平,与60例同年龄组健康成人对照,观察其动态变化。结果:VBI患者早期血浆NSE和ET含量较对照组明显升高(P<0.01),且随时间延长,二者含量均逐渐降低,NSE水平2d时达高峰,14d时基本恢复,而ET水平则始终高于对照组;与此不同,不同时间VBI患者血浆CGRP的水平均显著降低(P<0.01),但随时间的延长逐渐升高。结论:NSE、ET和CGRP与VBI的发生、发展密切相关;NSE的监测为VBI的早期诊断提供了可能,而ET、CGRP水平的变化对VBI的早期预防、早期治疗具有一定的指导意义。Objective To investigate the clinical significance of the dynamic changes of endothelin （ET）, calcitonin - gene related peptide （CGRP） and neuron - specific endolase （NSE） levels in patients with vertebro - basilar arterial insufficiency. Methods Plasma ET, CGRP and NSE levels were measured repeatedly with RIA in 120 patients with vertebro - basilar arterial insufficiency within the day of onset, on d2, d14 and d28 as well as in controls. Results In the patients, the plasma levels of ET and NSE were significantly higher than those in the controls during the early stage of disease （P 〈 0.01 ）. The levels declined gradually： with NSE, the levels were highest on d2 and returned to normal on d14, but the ET levels remained higher than normal on d28. On the contrary, the plasma CGRP levels were significantly lower than those in controls at the onset of disease and though increased gradually, remained significantly lower （P 〈 0.01 ）. Conclusion In patients with veretebro - basilar arterial insufficiency, the early rise of plasma NSE levels might be diagnostic, while the plasma ET and CGRP levels were closely related to the progress of the disease.
