羊膜移植小梁切除术的应用

摘　　要：目的 探讨羊膜移植小梁切除术治疗原发性闭角型青光眼的疗效评价。方法 50例63眼原发性闭角型青光眼患者，随机分治疗组25例36眼和对照组25例27眼，治疗组行羊膜移植小梁切除术，对照组行单纯小梁切除术．对术后前房、眼压、滤过泡、手术成功率及视力进行对照分析，随访6个月。结果 浅前房发生率：治疗组为13．9％，对照组为48．5％，两组之间差异有统计学意义（P〈0．01）。眼压：两组术前与术后不同时期眼压比较有明显差异（P〈0．05）．术后3个月、6个月两组眼压比较差异有统计学意义（P〈0．05）。滤过泡：术后2周功能性滤过泡治疗组占97.2％，对照组占74．1％；术后6个月分别为91．7％和62．9％（P〈0.05）。手术成功率：术后2周治疗组为97.2％．对照组为81．5％；术后6个月分别为91．7％和62．9％（P〈0．005）。术后视力：术后6个月对照组视力下降有10眼（37．1％），而治疗组只有4眼（11．1％）（P〈0.025）。结论 羊膜移植小梁切除术减少了手术早期并发症，提高了手术成功率。Objective To discuss the value of amnion transplantation in trabeculectomy for primary angleclosure glaucoma. Methods 63 eyes of 50 cases primary angle-closure glaucoma were randomly divided in to treatment group（36 eyes of 25 cases） and component group（27 eyes of 25 cases）. The patients of the treatment group were undergone trabeculectomy with amnion transplantation. The others were done trabeculectomy only. The depth of anterior chamber.ntraoeular pressure （IOP）, belbs, success rate and visual acuity were observed after operation. The follow-up was 6 months. Results The difference of shallow anterior chamber rate, lOP,functional bleb,success rate and visual acuity before and after operation between two groups was significant. Conclusion The use of Amnion transplantation could decrease the complication of trabeculectomy during the early postoperative period,and improve the success rate of operation.
