摘 要:目的对比研究添加谷胱甘肽的改良低钾右旋糖酐液(LPDG液)和低钾右旋糖酐液(LPD液)对离体肺的保护作用。方法健康杂种家兔18只,建立离体肺再灌注模型。随机分为正常对照组(n=6)、LPD液保护组(n=6)和LP-DG液保护组(n=6)。正常对照组供肺取出后直接离体灌注2 h,另外两组供肺分别以4℃LPD液和LPDG液灌洗后保存8h,再循环灌注2 h。灌注期间每隔1 h分别测定流入离体肺的静脉血及流出离体肺的动脉血的氧分压(PO2)和二氧化碳分压(PCO2);测定丙二醛(MDA)、超氧化物歧化酶(SOD)、肿瘤坏死因子(TNF-α)和白介素-8(IL-8)含量并计算肺湿干重比(W/D)。结果再灌注1 h和2 h后,LPDG液保护组流出血PO2均值显著高于LPD液保护组(均P<0.05),PCO2均值显著低于LPD液保护组(均P<0.05);LPDG液保护组W/D均值及MDA含量显著低于LPD液保护组(P<0.05),SOD活性均值显著高于LPD液保护组(P<0.05),TNF-α和IL-8含量则明显低于LPD液保护组(均P<0.05)。结论添加谷胱甘肽的改良低钾右旋糖酐液对离体肺的保护作用优于低钾右旋糖酐液。Objective To investigate the effect of improved low-potassium dextran solution added glutathione (LPDG) on isolated lungs preservation. Methods The reperfused models of isolated lung were established. Eighteen rabbits were randomly divided into three groups( n = 6) : normal control group, preserved by LPD solution group(LPD group) and preserved by LPDG solution group( LPDG group). In normal control group the lungs were reperfused immediately after being harvested (no flushing and storage). The lungs were flushed and stored with LPD solution or with LPDG solution at 4℃. After 8 h preservation,the lungs were ventilated and reperfused with fresh venous blood at a rate of 60 ml·min^-1 for 2 h. PO2 and PCO2 of venous blood and arterial blood were determined at 1 h intervals during the course of cyclic perfusion. MDA, SOD, TNF-α and IL-8 of lungs were measured at the end of perfusion and lung wet/dry weight ratio was also calculated. Results The arterial blood from isolated lungs in LPDG group showed significantly higher PO2 and lower PCO2 than those in LPD group after 1 h and 2 h of reperfusion(P 〈0. 05). LPDG group showed significantly lower Wet/Dry ratio and the amount of MDA than those in LPD group (P 〈 0.05 ), while the activity of SOD in LPDG group was higher than that in LPD group ( P 〈 0.05). TNF-α and IL-8 of lungs in LPDG group showed significantly lower than those in LPD group (P 〈 0.05).Conclusion LPDG solution provide superior lung function after 8 h ischemic time compared with LPD solution.
