不同Nevin分期原发性胆囊癌的超声诊断

摘　　要：目的:探讨超声影像在不同Nevin分期原发性胆囊癌的诊断价值.方法:对我院近10a经手术和病理确诊的553例原发性胆囊癌病例进行回顾性超声影像分析.结果:对Nevin分期中的Ⅰ和Ⅱ期原发性胆囊癌超声诊断符合率为零,Ⅲ期、Ⅳ期、Ⅴ期原发性胆囊癌的超声诊断符合率分别为3.61%,50.00%,99.49%.结论:超声对NevinⅠ和Ⅱ期原发性胆囊癌难以做出准确诊断;对Ⅲ期原发性胆囊癌的诊断率较低,但对囊壁局限性增厚伴有囊内结石时需要警惕:对Ⅳ和Ⅴ期原发性胆囊癌具有较高的诊断符合率.AIM： To evaluate the values of ultrasonography in the diagnosis of gallbladder cancer with different Nevin stages. METHODS： The ultrasonographic characteristics of 553 gallbladder cancer cases proved by surgery and pathological diagnosis were retrospectively analyzed. RESULTS： No cases with gallbladder cancers of Nevin stage Ⅰ and Ⅱ were diagnosed by ultrasonography, while the diagnostic accuracy was 3.61%, 50.00%, 99.49%, respectively, for the stage-Ⅲ, -Ⅳ, and - Ⅴ gallbladder cancers. CONCLUSION： Ultrasonography can hardly diagnose gallbladder cancers of Nevin stage Ⅰ and Ⅱ. For the stage-Ⅲ cancers, ultrasonography presents a low diagnostic accuracy, but for the stage-Ⅳ and -Ⅴ cancers, ultrasonography shows a relatively higher one.
