Operation for cosmetic improvement of baggy lower eyelids through an oral approach

摘  要:目的探讨对于睑袋隆起位于下1/3无须切除皮肤的中青年患者,采用口腔入路进行睑袋整形术的可行性。方法眶下神经阻滞麻醉,口腔上牙前庭沟切口,在骨膜下分离到眶下孔,保护眶下神经。从眶下神经两侧到眶下缘,打开眶隔,脂肪即膨出。10例患者20侧采用此方法,其中1例患者曾有下眼睑外伤史。2例患者曾行睑袋整形术,但睑袋处皮肤不平整,特别是靠近眶下缘处,仍隆起而再次手术。结果所有患者睑袋均得到平复,下睑因未受到手术影响(不损伤下睑提肌),静态和动态时,与邻近组织和谐、自然。睑袋与眶下区皮肤自然过渡,而且平坦。切口愈合良好,无感染。有5例眶下区麻木,3个月内全部恢复。有1例重度麻木,6个月后完全恢复。结论本术式对于睑袋位于下1/3的中青年患者,是一种不留皮肤瘢痕的较好选择。Objective To explore an esthetic operation for improvement of baggy of lower eyelids by an oral approach. Methods Ten patients aged 27-38 underwent this operation. Under block anesthesia of the infraorbital nerve, through an incision of the oral cavity vestibule, the infraorbital nerve cotdd be seen after dissection of the forarnen infraorbitale under the periosteum. The orbital diazoma was opened from the bilateral infraorbital nerve to the infraorbital border, and part of the periorbital fat was then removed through the oral approach. Resuits Good cosmetic results were obtained for all patients with disappearance of baggy lower lids. Lower eyelids were natural and harmonious with surrounding tissues in both dynamic and static states and the lower eyelid eleva- tor muscle was without injury. There was natural skin transition between the baggy part of the eyelid and the infraorbital region. The oral incision healed without infection. In five cases there was a numb sensation in the infraorbital region, but this resolved within 3 months. One patient had a more severe sensation of numbness which also resolved completely after 6 months. Conclusion This procedure may be a good choice for patients without excessive eyelid skin to correct bagginess of lower eyelids through the oral approach.
