血清糖化白蛋白与内源性分泌型糖化终末产物受体比值对糖尿病并发冠心病危险的评估价值

摘　　要：目的测定糖尿病及糖尿病并发冠心病患者血清糖化白蛋白(GA)和内源性分泌型糖化终末产物受体(es-RAGE)水平,探讨GA与esRAGE比值对判断糖尿病并发冠心病和冠状动脉病变严重程度的价值。方法入选单纯2型糖尿病患者64例(糖尿病组),2型糖尿病合并冠心病患者70例(糖尿病合并冠心病组),并根据冠状动脉病变支数分为三个亚组:一支病变组(22例),两支病变组(19例)和三支病变组(29例)。以42例健康者为对照(对照组)。测定各组血清GA和esRAGE水平,计算GA/esRAGE比值。结果糖尿病合并冠心病组GA(21.30±5.22)%和GA/esRAGE比值(2.03±2.26)明显高于单纯糖尿病组(19.11±4.45)%和(1.12±1.44)(均P<0.05)及对照组(14.13±1.38)%(P<0.01)和(0.90±1.77)(P<0.05),而糖尿病合并冠心病组esRAGE(0.21±0.17)明显低于单纯糖尿病组(0.32±0.30,P<0.05)和对照组(0.56±0.57,P<0.01)。GA/esRAGE比值与冠状动脉病变支数显著相关(rs=0.474,P<0.001)。结论GA与esRAGE比值对判断糖尿病时冠状动脉病变严重程度具重要临床价值。Objective To investigate the clinical significance of ratio between glycated albumin （GA） and endogenous secretory receptor for advanced glycated end products （esRAGE） in assessing the presence and severity of coronary artery disease in patients with type 2 diabetes meUitus. Methods Senma levels of GA and esRAGE were measured and GA/esRAGE ratio was calculated in 70 patients with diabetes and coronary artery disease,64 with type 2 diabetes only,and 42 controls.Results Serum level of GA（21. 30±5.22）and GA/esRAGE ratio （2.03 ± 2.26）in diabetic patients with coronary artery disease were significantly higher than those in patients with diabetes only （19.11 ±4.45 and 1.12 ± 1.44, all P 〈 0.05） and in controls （14.13 ± 1.38 and 0.90 ±1.77, all P 〈 0.05） .Serum level of esRAGE（0.21 ±0.17） in diabetic patients with coronary artery disease was significantly lower ihan in patients with diabetes only （0.32±0.30, P 〈 0.05） and in controls（0.56 ±0.57, P 〈 0.01）. GA/esRAGE ratio correlated signitlcandy with number of diseased vessel （spearman correlate coefficient rs = 0.474, P 〈 0.001 ）. Conclusion GA/esRAGE ratio may be a valuable parameter for predicting the presence and severity of coronary artery disease in patients with type 2 diabetes.
