微小乳腺癌的直接数字化X线征象与临床查体及病理关系的分析

摘　　要：目的分析微小乳腺癌的直接数字化X线征象,并探讨其与临床查体及病理的关系。方法回顾性分析经手术、病理证实的微小乳腺癌68例。采用直接数字化乳腺X线机后处理工作站屏幕阅读方式。结果X线征象为结节者39例,单纯钙化14例,结构紊乱8例,致密影7例,除单纯钙化外,其他征象伴有钙化者37例。临床触诊阳性42例,阴性26例。本组15例(占22%)为非浸润性癌,不可触及微小癌中46%为非浸润性癌。结论微小乳腺癌X线表现为结节、单纯钙化、结构紊乱或致密影,细钙化是微小癌重要X线征象。直接数字化X线摄影对诊断临床触诊阴性的微小乳腺癌有重要价值。Objective To analyze the features on direct digital mammography of minute breast cancer, and investigate relationship between the features, clinical examination and pathology. Methods The mammographic findings of 68 patients with pathologically proved minute breast cancer were retrospectively analyzed. The X diagnoses were done by screen reading of workstation of direct senographe. Results Nodes were found on direct digital mammography in 39 cases,isolated microcalcifications in 14 cases,architectural distortion in 8 case and asymmetrical increase of local density on mammngram in 2 cases. Thirty - seven cases of microcalcifications were found except isolated microcalcifications. There were 15 cases （22 % ） of ductal carcinoma in situ, and 46 % of nonpalpable minute cancers were infiltrating carcinoma. Conclusion The X ray features of minute cancer include nodes, isolated clustered microealcifictions, local architectural distortion, asymmetry local dense area. Mierocalcifiction is an important sign of minute breast cancer. Direct digital mammography has important values in the diagnosis of nonpalpable minute breast cancer.
