摘  要:目的探讨CT灌注成像在兔肝纤维化模型中的应用价值。方法70只日本大耳白兔,实验组62只,每周一次四氯化碳灌胃,根据肝功能指标来调整四氯化碳剂量,分别在给药8、12、16、20周处死8只;对照组共8只,在8、12、16、20周各处死2只。所有动物于处死前行CT灌注扫描,绘制感兴趣区时间-密度曲线(TDC),计算HAP、HPP、HTP、HPI。以病理结果为标准进行肝纤维化分组。结果随着肝纤维化程度加重,肝门脉灌注量呈下降趋势,而肝动脉灌注量呈上升趋势。统计显示S3、S4期HPP、HTP与对照组比较有统计学差异。S4期HPI与对照组、S1期HPI有统计学差异。结论CT肝脏灌注扫描能反映肝纤维化各期的血流动力学变化趋势。参照肝脏血流灌注参数可对晚期肝纤维化、早期肝硬化进行早期诊断,并对观察肝纤维化治疗效果有重要价值。Objective To explore the value of CT perfusion imaging in early diagnosis of liver fibrosis in adult rabbits. Methods Liver fibrosis was induced in rabbits (n=62) by intragastric administration of CCl4 once a week. The doses of CCl4 were modified according to the index of liver function. Controls received equivalent water. Eight rabbits in experiment group and 2 rabbits in control group were killed at 8,12,16,20 weeks respectively. Before animals were sacrificed, CT perfusion scan were performed, then functional maps and perfusion parameters including HAP, HPP, HTP, HPI were computed and analyzed. Each liver lobe was processed for histological examination, which was the basis of pathologic classification of liver fibrosis. Results With the progress of liver fibrosis, HPP decreased whereas HAP increased. The differences of HPP and HTP between S3 stage,S4 stage and control group were significant. HPI in S4 stage was significantly different from that of S1 stage and control group. Conclusion CT perfusion imaging can demonstrate hemodynamic trend in different liver fibrosis stage. Hepatic perfusion parameters measured with CT can evaluate the severity of advanced liver fibrosis or early stage hepatic cirrhosis. The data of CT perfusion, especially the changes should be valuable for early clinical treatment and following up.
