论火郁发之——从《伤寒论》探析火郁发之的机理

摘　　要：通过《伤寒论》原文的分析来探析火郁发之的含义及机理,火郁可有外因和内因两大类致病因素,导致邪热郁遏在里。其病机是气滞于内,阳郁于里,不能透达所致,治疗宣畅气机,透达郁阳,有火郁发之之意也。实证火郁重在祛除有形实邪,给郁火以出路,即是“火郁发之”。《伤寒论》不仅论述了气分火郁证治,还论述了血分火郁证的证治。虚证火郁可用补法治疗,例如因虚致郁可用小建中汤治疗。《伤寒论》中的吴茱萸汤证亦可理解为中阳亏虚,虚火内郁之证。可见,发之,不限于王冰所说的:“发汗”之意,火郁发之在《伤寒论》中是指种种调动机体正气,祛邪外出的治则。Discussing the definition and mechanism of emission therapy through the analysis of the original text of Treatise on Febrile. Both internal and external factors, which may block the emission of heat out of the body, would cause fire stagnancy. The pathogenesis of fire stagnancy is due to the internal stagnation of Qi and fire. Therefore, based on the pathogenesis, the emission therapy of fire stagnancy is to smooth the activities of Qi and release the stagnated fire. The treatment of excess syndrome of fire stagnancy, also known as ＂the emission therapy of fire stagnancy＂, is to eliminate pathogenic factors and disperse stagnated fire. Both of the treatments for Qi phase and blood phase fire stagnancy are recorded in Treatise on Febrile. The deficiency syndrome of fire stagnancy could be treated by tonic. For instance, Xiao Jian Zhong Tang could be applied as the treatment for fire stagnancy resulted from deficiency. Wu Zhu Yu Tang, recorded in Treatise on Febrile, is also another treatment for stagnation of deficiency fire. As a. result, the emission therapy is not restricted to ＂perspiration＂ mentioned by Wang Ping. According to the Treatise on Febrile, the emission therapy of fire stagnancy is a therapy to strengthen healthy Qi and eliminate pathogenic factors.
