胃康胶囊对胃溃疡患者T细胞亚群及白细胞介素2和6水平的影响

摘　　要：[目的]观察胃康胶囊对胃溃疡(GU)患者T细胞亚群及血液和组织中白细胞介素2(IL-2)、白细胞介素6(IL-6)水平的影响,进一步探讨其抗溃疡的作用机制。[方法]将符合诊断标准的88例GU患者,采用双盲双模拟研究方法随机分为治疗组(胃康胶囊)45例和对照组(丽珠得乐)43例,用流式细胞术、放射免疫法、ELISA法分别检测血液T细胞亚群、血清及溃疡灶周围黏膜组织中IL-2、IL-6的水平,并与15例健康人作比较。[结果]GU患者IL-2水平低下,IL-6水平升高,与健康人比较P<0.01。胃康胶囊治疗后能使患者CD3+、CD4+、CD4+/CD8+比值恢复正常,明显优于对照组。2组治疗后血清IL-2、IL-6均有较好改善,但不及溃疡灶周围黏膜组织中IL-2、IL-6的改善明显(P<0.01);治疗后治疗组血清及组织改善优于对照组(P<0.05、<0.01)。[结论]GU患者免疫功能低下,存在IL-2的欠表达和IL-6的过表达。胃康胶囊能有效地抑制溃疡及溃疡复发,可能与其提高患者免疫功能,调节血清及溃疡灶周围黏膜组织中IL-2、IL-6的水平有关。[Objective]To observe effect of Weikang capsule on T cell subgroup and changes of IL-2, IL-76 in blood and tissue in patients with gastric ulcer and to explore its mechanism of treating gastric ulcer. [Methods]Eighty eight cases were taken as observed subjects,double-blind and double dummy design method was used to observe the therapeutic effects of Chinese drug. Eighty eight cases were randomly divided into treatment group （45 cases, treated by Weikang capsule） and control group （43 cases, treated by Lizhudele capsule）. The blood were determined by flow cytometry, radioimmunoassay and ELISA for T cell subgroup , IL-2 and IL-6 concertration in serum and mucosal tissues around ulcers. Cases were compared to 15 healthy populations. [Results] The level of IL-6 and IL-2 of patients with gastric ulcer were lower than those of the healthy persons（P〈0.05, P〈0.01）. The level of CD^3＋, CD^4＋ , CD^4＋/CD^8＋ were recovered to normal after treatment by Weikang capsule, Lizhudele had no such effect. The level of IL-2, IL-6 in serum were significantly improved, but lower than those in mucosal tissues around ulcers after treatment（P〈0. 01）. The level of IL-2,IL-6 in serums and tissues in treating group was significantly higher than that in control group after treatment（P〈0.05,〈0. 01）. [Conclusion ]The immunologic function of the gastric ulcer patients decreased , existed unexpression of IL-2 and overexpression of IL-6. Weikang capsule can efficiently restrain ulcer and ulcer recrudesce, and improve the immunologic function of the body, regulate the level of IL-2, IL-6 in serums and mucosal tissues around ulcers.
