摘　　要：目的探讨治疗先天性眼球震颤伴垂直代偿头位的手术矫正方法。方法1998～2006年在我院接受治疗的先天性眼球震颤伴有垂直代偿头位患者12例,根据下颌内收或上抬的垂直头位测定中间带位置和头位扭转角度,采用双眼垂直肌后退和缩短术进行中间带移位。结果12例患者中(下颌上抬7例,下颌内收5例)有9例术后代偿头位完全改善,3例术后仍残留10°以上的垂直头位。结论通过双眼垂直肌等量后退和缩短进行中间带移位可以明显改善先天性眼球震颤的垂直代偿头位,并能提高正前方双眼视力。Objective To evaluate the surgery for congenital nystagmus with a vertical compensatory head posture. Methods Twelve cases of congenital nystagmus with vertical plane torticollis received bilateral vertical rectus muscle recessions and resections based on the degrees of their chin elevation or chin depression. Results Among all cases （7 cases with chin elevation, 5 cases with chin depression） , The head posture was eliminated completely in 9 cases, and had residua of above 10 degree in 3 cases. Conclusion Surgery of bilateral vertical rectus muscle recessions and resections can improve the vertical plane torticollis of congenital nystagmus effectively so as to improve visual acuity in primary position.
