肝硬化时血清透明质酸层粘蛋白人 Ⅲ 型前胶原 Ⅳ 型胶原含量的变化

摘　　要：目的：研究肝炎后肝硬化及酒精性肝硬化患者血清中透明质酸（ＨＡ）、层粘蛋白（ＬＮ）、人Ⅲ型前胶原（ｈｐｃ：Ⅲ）、Ⅳ型胶原（Ⅳ、ｃ）四项肝纤维化血清学指标含量的变化及临床意义。方法：采用放射免疫法（ＲＩＡ）联合检测已确诊的肝硬化患者血清中ＨＡ、ｈｐｃ：Ⅲ、Ⅳ．ｃ含量与肝细胞功能指标、肝纤维化严重程度（Ｃｈｉｌｄ分级）进行比较、分别作相关性分析。结果：肝炎后肝硬化、酒精性肝硬化患者血清中ＨＡ含量明显高于正常人，差异有显著性（Ｐ＜０．０５），其中酒精性肝硬化患者血清中ＨＡ含量显著增高是肝炎后肝硬化的１．４８倍。在肝炎后肝硬化和酒精性肝硬化患者中同时分别伴有Ⅳ．ｃ与ＨＰＣ：Ⅲ含量的升高，且与肝纤维化程度、门脉高压，Ｃｈｉｌｄ分级呈正相关（Ｐ＜０．０５），与肝细胞功能指标相比较，结果无统计学意义（Ｐ＞０．０５）。结论：联合检测肝硬化患者血清中ＨＡ、ＬＮ、ｈｐｃ：Ⅳ．ｃ含量，可以反映肝硬化患者在不同临床阶段的肝损害程度和肝纤维化的活动，较常规肝功能指标敏感，是判断慢性肝病患者肝硬化发展趋势的良好指标。AIMS To investigate the change of hyaluronic acid (HA),Laminin(LN),human procollagen typeⅢ (hpc:Ⅲ) and Type Ⅳ collagen (Ⅳ.c)in the serums of the patients with hepatic liver cirrhosis (HC)and alcoholic liver cirrhosis (ALC) and their relations to the liver diseases.NETHODS HA,LN,hpc:Ⅲ and Ⅳ.c in the serums of the patients with liver cirrhosis were measured with RIA,compared with indexes of liver cell functions and seriousness levels (Child Classification) of hepatic fibrosis and made an analyse of their relationship respectively.RESULTS The contents of HA in the serums of the patients with hepatic cirrhosis and alcoholic liver disease are higher than that of the normal Man.There is a significant difference (P<0.05).And the content of HA of alcoholic liver disease is as 1.48 times high as that of hepatic cirrhosis.The contents of Ⅳ.c and hpc.Ⅲ in the serums are increased.They are correlated with the levels of hepatic fibrosis and high pressure of portal vein (V.P) and Child classification (P<0.05).It is shown that they are not related with the liver cell functions,that is to say there is no of statistics(P<0.05). meaning CONCLUSIONS It is shown that the damage level of liver and the activgity of hepatic fibrosis by the change of content of HA,LN,hpc:Ⅲ and Ⅳ.c measured by RIA.It is more sensitive than any other methods.It is a good method to check the liver diseases.
