卒中单元病房综合治疗脑卒中后抑郁状态的疗效

摘　　要：目的:探讨卒中单元病房综合治疗对脑卒中后抑郁状态的疗效。方法:随机抽取卒中单元病房的脑卒中患者450例为卒中组和普通神经内科住院的脑卒中患者180例为对照组,均采取常规药物治疗;卒中组同时配合综合康复治疗,包括肢体运动康复、语言训练、心理康复和健康教育等。将2组患者的PSD发生率进行比较,对有PSD患者进行治疗,并比较疗效。结果:治疗2周后,卒中组患者PSD的发生率低于对照组(P<0.01);治疗8周后,卒中组PSD患者汉密尔顿抑郁量表(HAMD)评分与治疗前及与对照组PSD患者比较均明显降低(P<0.01)。结论:在卒中单元病房综合康复治疗能降低卒中后PSD的发生率,并能显著改善PSD患者HAMD评分,提高临床疗效。Objective： To study the effect of comprehensive rehabilitation programs on the post-stroke depression of stroke patients in stroke unit. Methods： In stroke unit, 530 cases of acute stroke were randomly divided into rehabilitation group and control group. In rehabilitation group, the patients were intervened with comprehensive rehabilitation programs, while those in control group only with the ordinary medical drugs. The post-stroke depression indexes were used to assess the post-stroke depression and HAMD scores before and 8 weeks after therapy, respectively. A comparison in the incidence of post-stroke depression was carried out between the two groups. Results.. After 8 weeks, the scores and the incidence of post-stroke depression in the rehabilitation group were much lower than in the control group （P〈0.01）. Conclusion： Comprehensive rehabilitation programs in stroke unit can effectively lower the incidence of post-stroke depression and reduce the HAMD scores.
