中西药结合对老年胃溃疡愈合演化及其生存质量的影响

摘　　要：目的:观察中西药结合治疗对老年胃溃疡患者溃疡愈合的作用及其生命质量影响的可能机理。方法:老年胃溃疡患者56例,按照序数分为A、B2组各28例,均给予根除幽门螺杆菌(HP)治疗1周后,坚持口服西药奥美拉唑治疗;A组并增加中药胃乐舒冲剂,均观察2个月。结果:治疗2周时与治疗前比较,A、B组溃疡深层破损长度缩小率分别为64.3%和25.9%(P<0.05);治疗2个月后溃疡疤痕总S期和S2期获得率2组均差异无显著性意义,但生存质量3个维度的改善度A组高于B组(P<0.01)。随访6和9个月,溃疡复发率A组明显低于B组(P<0.01)。结论:中西药结合治疗老年胃溃疡,具有保护胃粘膜,降低不良反应率等作用,且长期疗效显著,患者生存质量提高。Objective： To observe the healing course of gastric ulcer and life quality in the elderly treated with Chinese and Western medicine. Method：Fifty-six old patients with gastric ulcer were divided into two groups： the group A was treated with Weileshu and omeprazole, and the group B with omeprazole only for 2 months. Two weeks, 2 months, 6 months and 9 months after treatment the healing evolvement was evaluated. A SF-36 questionnaire was filled in 2 months after therapy. Results：After 2 weeks of treatment, deep ulcers were reduced by 64.3% （18/28） and 25.9% （7/28） in length in groups A and B respectively with the difference being significant （P〈0.05）. Two months after treatment, there was no significant difference in the healing rate between two groups, but the improvement in three dimensions of life quality was better in group A than in group B （P〈0.01）. After follow-up for 6 months and 9 months, the ulcer recurrence rate in group A was significantly lower than in group B （P〈0.01）. Conclusion： In the treatment of the elderly ulcer, integrated Chinese medicine and Western medicine could protect gastric mucosa, lower rate of adverse reactions, significantly improve the long-term effectiveness and increase the quality of life of the patients.
