肺鳞状细胞癌肺泡腔隙充填生长的意义

摘　　要：目的探讨肺泡腔隙充填型生长方式在肺鳞状细胞癌的临床病理意义。方法观察117例肺鳞状细胞癌手术切除病例，观察肺泡腔隙充填型和肺泡腔隙充填为主型肺鳞状细胞癌的病理组织学特点，比较与其他类型肺鳞状细胞癌生存率的差异。结果117例肺鳞状细胞癌中包括肺泡腔隙充填型2例，肺泡腔隙充填为主型18例。2例肺泡腔隙充填型患者均生存，肺泡腔隙充填为主型的生存率在混合型中显著高于其他亚型（x^2=6．9148，P=0．0315）。结论肺泡腔隙充填型周围型鳞状细胞癌预后良好，具有大量肺泡腔隙充填结构的其他肺鳞癌也具有较好的预后。Objective To explore the clinicopathological significance of growth pattern of alveolar space-filling of pneumonic quamous cell carcinoma. Methods A total of 117 surgically resected lung quamous cell carcinoma between 1988 and 2001 were reviewed . All cases had folow-up result .Based on the histologic invasion pattern ,the pneumonic quamous cell carcinoma was classified under three subgroups as follow： the infiltrative type, the alveolar space-filling type and the combined type. The combined type was grouped into three subtypes ： the alveolar space-filling dominant type , the infiltrative growth dominant type and the equilibrium combined type .The histopathologic characters of were observed and the survival rate for each type was compared. Results In 117 cases of pneumonic quamous cell carcinoma,there were 66 cases of the infiltrative type , 2 of the alveolar space-filling type and 49 of the combined type. 2 cases of the alveolar space-filling type were survive.In the combined type, the survival rate of the alveolar space-filling dominant type was significantly higher than those of the infiltrative growth dominant type and the equilibrium combined type （x^2=6.9148,P=0.0315 ）. Conclusions The growth pattern of alveolar space-filling of pneumonic quamous cell carcinoma is an exceptive expansile invasion.The alveolar space-filling type of pneumonic peripheral quamous cell carcinoma had most favorable prognosis . The other patients with much element of alveolar space-filling type had better prognosis too.
