准分子激光屈光性角膜手术前病人的筛选  

THE SIGNIFICANCE OF SCREENING FOR PATIENTS BEFORE EXCIMER LASER PHOTOREFRACTIVE SURGERY FOR THE CORNEA

摘　　要：目的探讨准分子激光屈光性角膜手术前病人筛选的临床意义。方法对要求行准分子激光治疗近视病例进行回顾性分析研究,观察手术前病人筛选与手术并发症之间的关系及病人对疗效满意程度。结果1000例病人(1 978眼)经过术前筛选,将其分为A、B、C组。A组938例(1 860眼,占94.0%)符合手术适应证,经过准分子激光原位角膜磨镶术、准分子激光角膜切削术、准分子激光上皮下角膜磨镶术治疗,术后裸眼视力均达到或超过手术前最佳矫正视力,角膜地形图等检查正常,无术后裸眼视力下降、眩光及夜间视力障碍等手术并发症,病人对手术疗效满意;B组35例(66眼,占3.3%)为手术禁忌证;C组27例(52眼,占2.6%)为手术相对禁忌证,病人经治疗眼部及全身情况正常或选择合适的手术后效果同A组。结论准分子激光屈光性角膜手术前病人的严格筛选对减少手术并发症和提高手术后病人的满意程度都有积极的意义。Objective To assess the clinical significance of patients screening before excimer laser photorefractive surgery for the cornea. Methods A retrospective analysis was performed, on patients who asked for exeimer laser for myopia, in terms of the interrelation between preoperative screening and postoperative complications, as well as the patients＇ satisfaction. Results Preoperative screening was conducted in 1 000 patients （1 978 eyes） who were divided into groups A, B, and C. In A group, 938 patients （1 860 eyes, 94.0%） were indicated for the surgery. All of the naked visual aeuities achieved or exceeded the best preoperatively corrected visual aeuities after exeimer laser in situ keratomileusis, xeeimer laser kerateetomy, or exeimer subepithelial keratomileusis. All the patients had a normal corneal topography, without visual acuity descending, dazzling and visual disorder at night, In group B, 35patients （66 eyes, 3, 3%） were contraindicated, In group C, 27 patients （52 eyes, 2, 6%） were relatively contraindicated, the outcome was similar to that of group A after screening and proper surgery, Conclusion Strict screening before operation is of great significance to lessen postopeative complications and raise the patients＇ satisfaction.
