肺鳞癌和肺腺癌c-erbB-2及bcl-2表达水平与其影像学表现相关性的初步探讨

摘　　要：目的:从基因水平探讨肺癌影像学表现的分子生物学基础。材料与方法:利用免疫组化技术和计算机图像分析系统,测定经手术病理证实之15例肺鳞癌及18例肺腺癌组织中癌基因 c-erbB-2及 bcl-2的表达水平,并分析其表达水平与影像学征象的相关性。结果:肺鳞癌的影像学征象中肿块的大小,边缘清楚率及肺门纵隔淋巴结肿大率与 c-erbB-2的表达水平呈负相关,与 bcl-2的表达水平呈正相关;而三级支气管受累率则与 c-erbB-2呈正相关,与 bcl-2呈负相关。肺腺癌肿块的大小、边缘光滑率与 c-erbB-2的表达水平呈负相关,其肺门和纵隔淋巴结肿大率、三级支气管受累率及胸膜凹陷率与 c-erbB-2呈正相关。结论:癌基因 c-erbB-2及 bcl-2的表达对决定肺鳞、腺癌的影像学表现具有重要的作用。Objective:To explore the molecular biologic basis of image manifestations of lung carcinoma.Materials and Methods: Immunohistochemical technique in combination with a computer assisted TV-image analysis system were used to measure the ex- pression level of c-erbB-2 and bcl-2 in 15 cases of lung squamous carcinoma and 18 cases of lung adenocarcinoma identified patho- logically.The expression of c-erbB-2 and bcl-2 in lung carcinoma and its image signs were analysed correlatively.Results:Some im- age signs of lung squamous carcinoma such as the size and well-defined rate of the mass,lymphadenopathy of hilum or mediastmum were correlated negatively with the expression level of c-erbB-2 but positively with that of bcl-2.Invasion of tertiary bronchus were correlated positively with the expression level of c-erbB-2,negatively with that of bcl-2.Some image signs such as the size and well-defined rate of adenocarcinoma mass were correlated negatively with the expression level of erbB-2,while lymphadenopathy of hilum or mediastinal invasion of tertiary bronchus and pleural indentation of adenocarcinoma were correlated positively with that of c-erbB-2.Conclusion:The expression of c-erbB-2 and bcl-2 plays an important role in the image manifestations of lung squamous carcinoma and adenocarcinoma.
