摘　　要：目的:探讨十二指肠病变(溃疡、炎症和肿瘤等)的 CT 表现及其检查技术。材料与方法:采用充分口服碘水、低张和及时扫描的 CT 检查方法,分析58例经手术病理证实的十二指肠病变的 CT 表现。结果:和胃镜及上消化道钡餐检查相比,CT 对十二指肠溃疡的诊断无显著价值,但对于穿透性溃疡,能发现其并发症。十二指肠炎症的 CT 表现为广泛性的肠壁增厚,肠腔可扩张,二次低张法能鉴别炎症性和肿瘤性肠壁增厚。十二指肠腺癌的 CT 表现为肠壁局限性增厚、腔内充盈缺损和龛影;1例平滑肌肉瘤表现为与十二指肠壁广基相接的肿块,有"牛眼征",和胰腺分界不清;1例十二指腺瘤表现为腔内息肉状充盈缺损。结论:CT 对十二指肠病变的诊断和鉴别诊断颇有价值。Objective:To investigate the CT manifestations and techniques of duodenal disordera(peptic ulcer,inflammation,tumor etc.).Materials and Methods;Sufficient oral contrast agents,antiperistaltic drugs and immediate CT scanning were needed tech- nologically.The CT findings of 58 cases of duodenal disorders proved by operation and pathology were analysed.Results:In com- parison to upper gastrointestinal barium examination and endoscopy,CT has little value in the diagnosis of peptic ulcer besides per- forative ulcer,of which CT can show the complicatios.On CT duodenitis was shown as diffusely thickened duodenal wall,but the lumen can be widened with double hypotonic method.Duodenal adenocarcinomas had the manifestations of local wall thickening, filling defect and crater.One case of leiomyosarcoma showed a large,ulcerated,exophytic mass,which had affected the pancreas. The intraluminal polypoid filling defect was found in one case of Brunner gland adenoma.Conclusion:CT is valuable in the diagno- sis and differential diagnosis of duodenal diseases.
