胆管癌表皮生长因子受体、增殖细胞核抗原的表达

摘　　要：为研究胆管癌组织中表皮生长因子受体（ＥＧＦＲ）、增殖细胞核抗原（ＰＣＮＡ）的表达及其生物学意义，并探讨ＥＧＦＲ表达与血型Ａ抗原的关系，通过应用免疫组织化学方法（ＡＢＣ法）检测３０例肝外胆管癌。结果发现１５例（５０％）为ＥＧＦＲ阳性，ＰＣＮＡ阳性７例（２３．３３％），对照组５例为非肿瘤胆总管，ＥＧＦＲ、ＰＣＮＡ均为阴性。胆管癌与非肿瘤胆总管组ＥＧＦＲ及ＰＣＮＡ表达率有显著差异，细胞分化差、有胆管外转移的胆管癌ＥＧＦＲ的表达率高于细胞分化好、无胆管外转移病例。胆管癌各组间ＰＣＮＡ表达率均无显著性差异。ＥＧＦＲ表达与血型Ａ抗原无关。结果提示：检测ＥＧＦＲ表达是判断胆管癌增生活性的有效手段，而ＰＣＮＡ尚需进一步研究。. Expression of edidermal growth factor receptor (EGFR ) and proliferating cell muclear antigen (PCNR ) in 30 cases of bile duct carcinoma and its rela- tionship to prognosis were analysed by immunohist tochemical study ( ABC immunoperoxidase method ) . with the controls of non-carcinoma common bile duct in 5 patients with cholelithiasis . and also studied the rela- tionship between EGFR and blood-group A antigen. EGFR positive expression and PCNA positive expression were found in 15 cases (50%) and 7 (23. 33%) out of 30 cases of bild duct carcinoma respectively , with the PCNA positive index being 5. 11 % - 1. 72%., while 5 controls common bile duct were all negative in EGFR and PCNA. The results showed the bile duct carcinoma with low tumor cell differentiated degree and (or )metas- tasis out of bile duct got high EGFR positive expressiion rate . but there was no significant differences between middte differentiated group and low differentiated group in EGFR expression positive rate. In PCNA positive in- dex , there were no significant difference concerning tumor cell differentiation and metastasis. Expression of EGFR had no ralation with blood-group A antigen. From above it was concluded that detection of EGFR on parafin embedded . tissue with immunohistochemical study was an effective method for .the judgement of bile duct carcinoma proliferating activity.
