基质金属蛋白酶-9及组织基质金属蛋白酶抑制剂-1在肝细胞癌中的表达及意义

摘　　要：目的：探讨基质金属蛋白酶-9（MMP-9）及组织基质金属蛋白酶抑制剂-1（TIMP-1）在肝细胞癌（HCC）中表达的意义。方法：收集HCC手术切除标本42例，采用免疫组化法检测MMP-9及TIMP-1的表达。结果：正常对照组中MMP-9、TIMP-1的表达均为阴性。MMP-9及TIMP-1在HCC中表达的阳性率分别为73．8％和69．0％，两者的阳性表达与患者年龄、性别、AFP是否阳性、HbsAg是否阳性、肿瘤大小、病理分级无关（P〉0．05）。有癌栓组MMP-9阳性率为100％，无癌栓组阳性率为63．3％，组间差异有统计学意义（P〈0．05）。包膜完整组MMP-9阳性率为33．3％，包膜不完整组阳性率为84．8％，组间差异有统计学意义（P〈0．01）。TIMP-1在以上2组间差异均无统计学意义（P〉0.05）。结论：MMP-9和TIMP-1的表达与HCC关系密切，检测HCC中两者的表达，可作为判断HCC侵润转移的指标。Objective To investigate the significance of MMP-9 and TIMP-1 expression in the HCC. Methods The expression of MMP-9 and TIMP-1 in 42 cases of HCC were detected by the immunohistochemical staining method. Results The expression of MMP-9 and TIMP-1 in normal controls were all negative. The positive rate of MMP-9 and TIMP-1 in the HCC were 73.8% and 69.0% respectively. There were no obvious relationships among MMP 9 or TIMP-1 with AFP, HbsAg, age groups, sex, size of tumor, pathologic grade （ P 〉 0. 05）. The expression of MMP-9 was related to poor encapsulating and portal embolus （ P 〈 0. 05）, while TIMP -1 was not related to them （ P 〉 0. 05）. Conclusions The expressions of MMP-9 and TIMP-1 were closely related to the HCC. It will be helpful to judge the ability of invasion or metastasis of HCC and predict the prognosis of patients by detecting the expression of MMP-9 and TIMP-1.
