摘 要:目的研究Toll样受体4(TLR4)896A>G基因错义突变与重型急性胰腺炎(SAP)胰腺坏死组织感染的相关性。方法采用错配PCR-限制性片段长度多态性(RFLP)分析方法在SAP伴胰腺组织坏死的病人和健康自愿者人群中检测TLR4(896A>G)的等位基因频率。结果1所有TLR4(896A>G)的基因突变均为杂合子;2SAP胰腺组织坏死伴胰腺感染组(50例)TLR4基因错义突变频率为20%,与健康志愿组TLR4基因错义突变频率5.6%相比,TLR4基因错义突变频率明显升高,差异有统计学意义(P=0.019);3在SAP伴感染病人中,TLR4基因错义突变组G-细菌感染率为44.1%(15/34),明显高于无TLR4基因错义突变组18.5%(15/81),差异有统计学意义(P=0.04)。结论TLR4(896A>G)基因错义突变与SAP胰腺坏死组织的感染相关联。Objective To determine the relationship between Toll like receptor (TLR) 4 (896A 〉G) mutations and pancreatic necrotic infection in acute pancreatitis (AP). Methods TLR4 (896A〉G) mutations were detected by mispairing PCR-RFLP analysis technique in the patients with pancreatic necrosis and healthy volunteers. Results ① All of the TLR4 mutations were heterozygotes; ②The G allele frequencies of TLR4 genes were significantly higher in the patients with pancreatic infection than in the healthy volunteers; ③The incidence of gram-negative infection was significantly higher in the patients with the TLR4 mutations [15(44.1%) of 34] than that in the wild type population [15(18.5%) of 81]. Conclusion TLR4 (896A〉G) mutations are associated with the infection of pancreatic necrosis in the patients with AP.
