摘　　要：目的探讨CT定位下半月神经节射频热凝治疗第Ⅰ+Ⅱ支受累的三叉神经痛的方法及其效果。方法先以定位架帮助,后在CT定位下采用卵圆孔前入路穿刺,局麻下行半月神经节射频热凝术。第Ⅰ支热凝温度和时间为65℃、67℃,各维持60 s;第Ⅱ支为70℃、75℃,各2～3 min。结果三叉神经第Ⅰ+Ⅱ支痛患者13例,全部病例术后当日三叉神经痛发作完全消失。随访3～16个月,无疼痛复发。结论定位架帮助,在CT定位下,经卵圆孔前入路穿刺,采用半月神经节射频热凝术治疗第Ⅰ+Ⅱ支受累的三叉神经痛是一种安全、疗效好的方法。Objective To explore the method and the effect of radiofrequency thermocoagulation with CT location for treatment of trigeminal neuralgia of the first and second branches. Methods Oval foramen was punctured with the help of localization cage and under the guidance of CT localization. Firstly local anesthesia was applied, then thermocoagulation temperatures for the first and second branchs were performed at 65℃ ,67 ℃for 60 s and 70℃ ,75℃ for 2-3 min respectively. Results Pain was fully eliminated in 13 cases after operation. There were no recurence during the follow-up period of 3-16 months. Conclusion Radiofrequency thermocoagulation with CT location for treatment of trigeminal neuralgia of the first and second branches is a safe and effective method.
