骨肿瘤手术前后骨型碱性磷酸酶的变化及临床意义

摘　　要：目的检测骨肿瘤患者手术前后血清中总碱性磷酸酶(TALP)和骨型碱性磷酸酶(BALP)的活性,研究TALP和BALP在骨肿瘤手术治疗前后的变化及治疗监测。方法采用法国临床化学推荐的酶联免疫吸附(ELISA)方法测定BALP和速率法测定TALP水平。结果恶性骨肿瘤组和转移性骨肿瘤组血清TALP和BALP明显高于正常对照组(P<0.01);手术治疗后1、2、3周血清TALP和BALP水平明显下降(P<0.01)。结论血清TALP和BALP均为骨肿瘤临床诊断、病情监测又一灵敏、简便的生化指标,且血清BALP的灵敏度和特异性高于血清TALP。Objective To determine the serum levels of total alkaline phosphatase（TALP） and bone - specific alkaline phosphatase（BALP） and to explore the changes and clinical value of TALP and BALP before and after operating on the patients with bone tumor. Methods Serum BALP was assayed by enzyme linked immune absorbance assay（ELISA） and TALP by SFBE rate method. Results The serum levels of TALP and BALP in malignant bone tumor group and malignant tumor metastasized to the bone group were significantly higher than those of the normal control group （ P 〈 0.01 ） ; the serum levels of TALP and BALP in 1 - 3 weeks of operation therapy significantly decreased （P 〈 0.01 ）. Conclusion Serum TALP and BALP are sensitive and convenient biochemical indexes which reflect clinical diagnosis and monitoring the effect of therapy about bone tumor , and serum BALP is more sensitive and specific than TALP.
