摘 要:目的比较星形细胞瘤局部脑血流量(rCBF)图与局部脑血容量(rCBV)图的表现差异并探讨其意义。资料与方法对29例星形细胞瘤患者(Ⅰ~Ⅱ级15例,Ⅲ~Ⅳ级14例),行CT平扫及灌注检查,得到时间-密度曲线(TDC),生成脑血流量(CBF)图、脑血容量(CBV)图,比较感兴趣区的rCBF和rCBV。结果24例星形细胞瘤rCBF图与rCBV图表现一致;5例局部区域的rCBF图与rCBV图表现不一致,呈现低rCBF、中等或高rCBV。结论星形细胞瘤rCBF、rCBV图表现可不一致,在术前分级和判定疗效时应予重视。Objective To compare the difference between rCBF and rCBV of astrocytomas and explore its clinical significance. Materials and Methods Both conventional and perfusion CT were performed in 29 patients with pathologically confirmed astrocytomas ( 15 cases in Ⅰ - Ⅱ grade, 14 cases in Ⅲ -Ⅳ grade). The time-density-curves (TDC) were obtained, and cerebral blood flow (CBF) maps and cerebral blood volume (CBV) maps were generated, the rCBF and rCBV of the region of interest (ROI) were compared. Results 24 astrocytomas exhibited match between rCBF and rCBV, 5 astrocytomas were mismatched with reduced rCBF and preserved or increased rCBV in ROI. Conclusion The difference between rCBF and rCBV in some area of astrocytomas is existed, rCBV and rCBF can be more useful in preoperative asessment of histological grading in astrocytomas and monitoring therapy effect.
