摘 要:目的探讨肺炎支原体(mycoplasma pneumoniae,MP)感染性中枢神经系统(central nervous system,CNS)疾病中细胞因子的特异性及可能的发病机制。方法25例MP性CNS感染患儿,按MP感染至CNS出现异常分为:早发组(≤7d)16例、迟发组(>7d)9例;采用ELISA法检测25例MP性CNS感染患儿、10例病毒性脑炎患儿和10名对照组儿童血浆和脑脊液IL-6、IL-8、IL-18、肿瘤坏死因子α(TNF-α)和转化生长因子β1(TGF-β1)水平并进行比较。结果IL-6和IL-8在MP和病毒性CNS感染的患儿血浆、脑脊液中均升高,与对照组比较差异均有统计学意义(P<0.05)。IL-18仅在迟发性MP性CNS感染组脑脊液中明显升高,与其他各组比较差异有统计学意义(P<0.05);而在血浆中,各组比较差异无统计学意义(P>0.05)。TNF-α和TGF-β1在各组血浆和脑脊液中水平与对照组比较差异无统计学意义(P>0.05)。结论IL-6、IL-8和IL-18可能参与MP引起的中枢神经系统感染,但前二者并不具有特异性;IL-18或许是MP引起的迟发性CNS感染较特异性指标;TNF-α和TGF-β1可能不参与其发病机制。Objectives To investigate the specificity of cytokines involved in central nervous system (CNS) infection caused by Mycoplasma pneumoniae (MP) and its possible mechanism. Methods Twenty-five children with CNS infection caused by MP were divided into two groups according to the on set time of CNS symptoms, early-onset (before 7 days, n = 16) and late-onset (after 7 days, n = 9). The levels of interleukin (IL) - 6, IL-8, IL-18, TNF-α and TGF-β1 in the blood and cerebrospinal fluid (CSF) were determined by ELISA in 25 children with MP-CNS infection, 10 children with viral encephalitis and 10 control children. Results Compared to the control groups, the levels of IL-6 and IL-8 were significantly high in both blood and CSF in children with CNS infection caused by MP (P 〈 0.05) . The level of IL-18 was only significantly high in CSF of the late-onset group (P 〈 0.05) and there was no significant difference in the blood among all the groups. No significant difference of TNF-α and TGF-β1 was detected in the blood and CSF. Conclusions IL-6, IL-8 and IL-18 might be involved in CNS infection caused by MP, while IL-6 and IL-8 have no specificity. IL-18 might be a specific index for the late-onset CNS infection caused by MP. TNF-α and TGF-β1 might not be associated with the pathogenesis of CNS infection caused by MP.
