放射性^(125)碘粒子近距离治疗20例中晚期恶性肿瘤近期疗效分析

摘　　要：目的探讨放射性125碘粒子组织间永久植入近距离治疗中晚期肿瘤的临床疗效。方法各种中晚期肿瘤20例采用组织间永久植入125碘粒子,在B超引导下或术中直视下植入粒子数3~70粒,活度14.8~25.9MBq/粒。植入术后1周内行X线或CT检查有无粒子迁移,在术后1个月行CT检查进行质量验证,在术后1、2、3、6个月后随访B超或CT,进行疗效评价。结果2个月后复查CT,近期总有效率85%。其中完全缓解(CR)15%(3/20),部分缓解(PR)70%(14/20),无变化(NC)10%(2/20),进展(PD)5%(1/20)。治疗后随访至今(2~6个月),所有治疗患者无一例出现由于放射治疗引起的并发症,患者的生存期在继续随访中。结论放射性125碘粒子组织间永久植入近距离治疗中晚期肿瘤的近期效果好,安全,副反应少,是一种新的治疗肿瘤手段。Objective To discuss short-term effect of intraoperative Iodine-125 seed interstitial permanent implantation brachytherapy of middle-late stage cancer. Methods Iodine-125 seed interstitial permanent implantation for 20 cases middle-late stage cancer. Intraoperative Iodine-125 seed implantation guided by ultrasound, the activity of ^125I per seed ranged from 14. 8- 25.9 MBq, the total number of sources implanter ranged from 3-70. All of the cases had routine chest X-ray or CT with in a week after implantation. The evaluation of post-implant quality was clone with CT 1 month after implantation. To evaluate efficacy with ultrasound or CT one, two, three, six months after implantation. Results Total effective rate in short time was 85%, including CR （completely relieved） 15% （3/20）, PR （partial relieved） 70% （14/20）, NC （no respone） 10% （2/ 20）, PD （progressive disease） 50% （1/20）. Follow-up from two to six months, all of the cases had no comlications of radiotherapy. The lifetime continues to follow-up. Conclusions Short-term effect of intraoperative Iodine-125 seed permanent implantation brachytherapy of cancer have satisfying clinical effects, low morbidity and high efficacy, a new method of treatmemt cancer.
