摘  要:目的探讨FQ-PCR检测宫颈癌旁盆腔组织中HPV-DNA对预测早期宫颈癌微小转移的意义。方法采用FQ-PCR方法,检测31例Ia-IIb期宫颈癌患者的原发灶及其癌旁组织(阴道切缘、宫旁切缘、盆腔淋巴结)中的HPV亚型和DNA拷贝数,并与患者术后组织病理学结果比较。结果31例癌旁组织HPV-DNA总检出率为58.1%,其中病理检测发生癌旁转移的7例患者均检测到HPV-DNA。宫颈原发灶、组织病理学阳性和阴性的癌旁组织HPV-DNA拷贝数分别为106.75±0.72,105.70±0.87和103.70±1.22,且它们之间有非常显著性差异(P<0.01)。结论癌旁组织检出HPV早于组织病理学发现转移,检测癌旁组织中HPV对早期微小转移具有提示意义,有助于发现组织病理学不易发现的早期隐匿性转移。Objective This study was to explore the significance of predicting early metastasis by human papillomavirus (HPV) DNA detection with fluorescence quantitive PCR (FQ-PCR) in adjacent cervical cancer tissue. Methods The subtypes and copies of HPV DNA were detected in thirty one cervical cancers and adjacent tissues ( including vagina] margin, parametrium and lymph node) By FQ-PCR, and the results were compared with pathological study. Results The positivity rate of HPV DNA was 51.8% in 31 adjacent tissue. HPV DNAs were all detected in adjacent tissue of 7 cases with pathologically confirmed cervical cancer metastasis. HPV DNA copies in adjacent cervical cancer tissue with and without pathological metastasis were 10^6.75 ±0.72, 10^5.70 ±0. 87 and 10^3.70±1.22, respectively, and the difference between them were significant (P 〈 0.01 ). Conclusion HPV DNA can be detected before pathological diagnosis of metastasis in adjacent tissue of cervical cancer. It could indicate early mild metastasis, and help to find early mild metastasis undetected by histopathology.
