摘　　要：目的探讨Survivin在胃癌组织中的表达及其与临床病理特征、恶性增殖的关系,并评价其预后价值。方法采用免疫组织化学技术(S-P法)检测50例胃癌组织及12例浅表性胃炎组织中Survivin和Ki-67的表达水平,应用Kaplan-Meier法进行生存分析。结果胃癌组织中Survivin阳性表达率为74.0%,显著高于浅表性胃炎组(P<0.001)。Survivin过表达与肿瘤浸润深度、淋巴结及远处转移、临床分期相关(P<0.05),而与胃癌组织分化程度无关(P>0.05)。Survivin和Ki-67表达有关联(P<0.05)。Survivin阴性表达者生存率显著高于阳性表达者(P<0.05),Cox比例风险回归分析表明Survivin是影响预后的独立因素(P<0.05)。结论Survivin在胃癌组织中过表达,在胃癌的恶性增殖中发挥作用,可作为预后不良的指标。Objective To investigate the expression of survivin in gastric carcinoma and its relationship with clinicopathological features and malignant proliferation, and to evaluate the prognostic value of survivin. Methods The expression of survivin and ki-67 was detected in 50 specimens from gastric carcinoma, 12 specimens from superfical gastritis by immunohistochemical technique （S-P）. Survival analysis was performed by Kaplan-Meier analysis. Results The positive rate of survivin in gastric cancer was 74.0%, significantly higher than that in superfical gastritis（P 〈 0.001 ）. The expression of survivin was significantly related with tumor infiltration depth, lymph node metastases , distant place metastases and clinical stage （P 〈 0.05 ）, but not with tumor cell differentiation（P 〉 0.05 ）. There was significant correlation between the expression of survivin and Ki-67 （P 〈 0.05 ）. Survival rate in survivin negative group was significantly higher than that in survivin positive group （P 〈 0.05 ）. Cox proportional hazard regression confirmed that survivin was a significant independent prognostic factor for survival （ P 〈 0.05 ）. Conclusion Survivin may play a role in the malignant proliferation of gastric carcinoma. The expression of survivin may indicate poor prognosis of gastric carcinoma.
