多烯他赛联合顺铂卡铂治疗宫颈癌49例分析

摘　　要：目的探讨多烯他赛联合顺铂/卡铂在宫颈癌新辅助化疗中的作用。方法选择2004年5月-2007年2月经病理确诊的Ⅰb2期宫颈鳞癌患者39例、Ⅱa期宫颈鳞癌患者10例,共49例,术前采用多烯他赛联合顺铂/卡铂的新辅助化疗,2-3周后手术。观察化疗一个疗程的有效率及术后宫颈深间质层浸润、脉管癌栓、淋巴结转移情况,同时观察新辅助化疗的毒副反应及评价新辅助化疗的安全性。结果49例中,完全缓解7例,部分缓解31例,稳定9例,进展0例,临床有效率为77.55%（38/49）;49例患者的肿瘤最大横径由化疗前的（4.46±0.31）cm缩小为化疗后的（3.08±0.22）cm,P〈0.01;49例患者手术后的宫颈深层间质浸润发生率为53.0%（26/49）;脉管癌栓发生率为30.6%（15/49）;淋巴结转移率为28.5%（14/49）。出现Ⅰ度-Ⅲ度的骨髓抑制48例,占97.96%（48/49）,经使用粒细胞集落刺激因子,骨髓抑制症状好转,血象恢复正常,无1例化疗相关死亡。结论多烯他赛联合顺铂/卡铂用于宫颈癌新辅助化疗,可缩小瘤体,提高宫颈癌的近期疗效。Objective To investigate the effect of Docetaxel combined cisplatin/ Carboplatin in neoadjuvant chemotherapy （NAC） on cervical cancer. Methods Totally 49 cases of cervical cancer in stage Ib2- Ⅱ a were diagnosed by pathology in our hospital from May 2004 to February 2007. All the patients were treated with one cycle of Docetaxel combined cisplatin/Cisplatin, followed by radical surgery. The pathology in two group and the immediate respond, deep interstitial infiltration, vascular invasion and toxicity in study group were observed to evaluate the effect and security of NAC. Results In study group, the rate of clinical response was 77.55% （38/49）,7 cases completely remission , 31 cases partly remission, and 9 cases stabilizing. The sizes of the tumors were diminished from （4.46±0.31）cm to （3.08±0.22）cm,P〈0. 01;the rate of deep interstitial infiltration was 53.0% （26/49）. the rate of vascular invasion was 30.6%（15/49） pathology characters and lymph node metastasis were 28.5%（14/49）. Conclusion Docetaxel combined cisplatin/Carboplatin in neoasjuvant chemotherapy （NAC） on cervical cancer can diminish the size of the tumors. It shows promoting clinical application.
