survivin和nm23基因表达与直肠癌浸润转移的相关性

摘　　要：目的探讨survivin、nm23基因表达水平与直肠癌分期、生存率、转移以及两基因表达的相关性,评价sur-vivin、nm23基因在直肠癌预后判断中的价值。方法应用免疫组化技术检测survivin、nm23基因在70例直肠癌组织中的表达,TUNEL法检测癌细胞凋亡指数(apoptosis index,AI),并分析survivin基因、nm23基因与直肠癌细胞凋亡指数、Dueks分期、生存率、转移之间的关系。结果survivin阳性患者5年生存率显著低于survivin阴性患者,伴远处器官转移的肿瘤中survivin阳性表达率显著高于无远处器官转移者(P<0.05),survivin阳性患者的AI与sur-vivin阴性患者的AI相比,二者之间有显著性差异(P<0.01)。nm23的表达与淋巴结转移、浸润深度有关。且sur-vivin的表达与nm23的表达在直肠癌组织中呈显著的反比关系。结论survivin蛋白的高表达和nm23蛋白的低表达对判断直肠癌预后、生存率及转移有重要参考价值。Objective To investigate the relationship between the expression of survivin, nm23 and the clinical pathological parameters in carcinoma of rectum,and evaluate it＇s significance in predicting the prognosis. Methods Immunohistochemistry method and terminal deoxynudeotidyl transferals- mediated dUTP nick end labeling（TUNEL） were used to detect the expression of Survivin and apoptotic cell in situ in carcinoma of rectum tissues, and normal tissues from 70 cases and retrospectively analyzed the relationship between survivin and nm23,Al（apoptosis index）, Dueks＇ stage ,survival rate and metastasis. Result Positive rate of survivin with distant metastasis was significantly higher than that without metastasis （P 〈 0. 05） ;and in 5 -year survival patients was lower than in patients within 5 years（P 〈0.05）. The average Al of patients with positive expression of survivin was lower than that of patients with negative expression of survivin （P 〈0.01）. There was a significant relationship between the expression of nm23 and the infiltrative depth, lymph node metastasis. The expression of survivin significantly inverse proportion with that of nm23. Conclusion The low level expression of nm23 protein and the high level expression of survivin protein may play an important role in predicting the carcinoma of rectum prognosis, survival and metastasis.
