基质金属蛋白酶-3及其抑制剂-1在食管鳞癌组织中的表达和临床意义

摘　　要：目的研究食管鳞癌(ESC)组织中基质金属蛋白酶-3(MMP-3)及其抑制剂TIMP-1的表达情况,并探讨MMP-3、TIMP-1与ESC侵袭的关系和临床意义,为临床寻找判断ESC侵袭性的可靠指标。方法采用免疫组织化学链霉素抗生物素蛋白-过氧化物酶(SP)法检测50例ESC患者和15名正常人的食管组织,观察并分析MMP-3及TIMP-1在ESC及正常食管组织中的表达,MMP-3及TIMP-1与ESC临床分期、病变部位、淋巴结转移的关系,MMP-3及TIMP-1在ESC中的表达及其相关性。结果MMP-3和TIMP-1在ESC中的表达率分别为86．0%和74．0%,且两者的表达呈正相关(r=0．290,P=0．041);MMP-3和TIMP-1在ESC组织中的表达均明显高于正常食管组织(P值均＜0．05)。MMP-3的表达与ESC的TNM分期有关(r=0．411,P=0．003);而TIMP-1则与之不相关(P＞0．05)。以MMP-3诊断ESC的灵敏度为86．0%,特异度为86．7%,误诊率为13．3%,漏诊率为14．0%,总符合率为86．2%。一致性检验的统计量k=0．651,说明有较好的一致性,但无统计学意义(P=0．105)。结论MMP-3和TIMP-1在ESC组织中的表达较其在正常食管组织中明显上调,MMP-3可作为判断ESC恶性程度的辅助指标,在ESC细胞增殖、侵袭中起重要作用。Objective To study the relationship between matrix metalloproteinase-3（MMP-3） and tissue inhibitor of metalloproteinase-1 （TIMP-1） in esophageal squamous carcinoma（ESC） and their clinical significance in diagnosis of ESC metastasis. Methods The expression of MMP-3 and TIMP-1 were determined in 15 normal esophageal tissues and 50 esophageal squamous carcinoma specimens by s-p immunohistochemistry technique. The relationship between MMP-3 and TIMP-1 with the clinical staging, location and lymphatic metastasis of ESC were analyzed. Results The positive rates of MMP-3 and TIMP-1 were 86.0% and 74.0% in esophageal squamous carcinoma, respectively, both much higher than that of normal esophageal tissues（γ = 0. 290, P = 0. 041）. The MMP-3 expression had a positive relationship with TIMP-1 （P 〈 0. 05）. The expression of MMP-3 was related to lymph node metastasis and clinical staging, but the expression of TIMP-1 was not relevant with them. There was higher invasion rate in patients positive of MMP-3 than that negative of it. Conclusion The expression of MMP-3 and TIMP-1 in esophageal squamous carcinoma is markedly higher than that in normal esophageal tissues. The high expression of MMP-3 and TIMP-1 can serve as predictor for malignant behavior of esophageal squamous carcinoma. MMP-3 may play an important role in proliferation and invasion of esophageal squamous carcinoma. （Shanghai Med J, 2007, 30： 431-434）
