椎管内先天性肿瘤的诊断与治疗——附135例分析

摘　　要：背景与目的:椎管内先天性肿瘤临床表现多样,治疗上也各不相同。本研究旨在总结椎管内先天性肿瘤的诊断与治疗方法。方法:回顾性分析2000年1月至2005年4月经手术和病理明确诊断的135例椎管内先天性肿瘤的病理类型、部位、临床症状和体征、影像学检查及手术治疗方法和疗效。结果:表皮样囊肿35例,脂肪瘤26例,肠源性囊肿24例,皮样囊肿24例,畸胎瘤19例,脊索瘤7例。全切39例,近全切除52例,大部切除44例。术后症状好转100例,无变化22例,加重13例。无手术死亡。结论:椎管内先天性肿瘤多为良性,MRI和显微外科技术的应用改善了椎管内先天性肿瘤的诊断和治疗。BACKGROUND ＆ OBJECTIVE： Intraspinal congenital tumors are a group of tumors. This study is to investigate diagnosis and microsurgical treatment for intraspinal congenital tumors. METHODS： Clinical data of 135 patients with intraspinal congenital tumors, treated in the department of neurosurgery Beijing Tiantan Hospital from January of 2000 to April of 2005, were retrospectively analyzed. RESULT： There were epidermoid cysts in 10 cases, lipoma in 26 cases, enterogenous cysts in 24 cases, dermoid cysts in 24 cases, teratomas in 19 cases and chordomas in 7 cases. Total removal of tumors was achieved in 39 cases, subtotal removal in 52, and partial removal in 44 cases. Postoperative neurological symptoms were improved in 110 patients, remained unchanged in 22, and deterioated in 13 cases. No operative death occurred. CONCLUSIONS： Most of the intraspinal congenital tumors are benign. MRI and microsurgery play a key role in the diagnosis and treatment of the tumors.
