乳腺癌保乳术后患者生活质量调查及护理对策

摘　　要：目的探讨乳腺癌保乳术后患者生活质量水平,分析提高生活质量的护理措施。方法随机选取41例保留乳房乳腺癌根治术病例作为调查组、48例乳腺癌改良根治术病例为对照组,选用癌症治疗功能评价系统-乳腺癌生活质量测评量表测定患者术后生活质量。结果调查组总体生活质量得分稍高于对照组,差别无统计学意义(P>0.05)。功能状况和与乳腺癌相关的其他因素指标,调查组分值高于对照组;情绪状况指标的分值调查组低于对照组,差别有统计学意义(P<0.05);在身体状况、社会?家庭状况、与医生关系调查组和对照组的差别无统计学意义(P>0.05)。结论保乳术患者术后总体生活质量并不明显优于改良根治术患者,其中功能状况和与乳腺癌相关的其他因素方面较改良根治术患者好,但情绪状况较差。需要对保乳术后患者的情绪状况进行针对性护理干预,以提高其总体生活质量。Objective To investigate the quality of life（QOL） of breast cancer patients treated with breast conservative surgery and study the better nursing measures for improving the life quality. Methods Forty-one breast cancer patients who received breast conservative operation were selected random and compared with forty-eight patients who received modified radical surgery. Postoperative QOL of breast cancer patients was evaluated according to the Functional Assessment of Cancer Therapy-Breast Quality of Life Instrument（V3.0）. Results The total QOL scores of the breast conservative operation group was a little higher than that of patients in the modified radical surgery treatment group without significant difference（P〉0.05）. The score of the emotional function in the breast conservative operation group was lower than that of patients in the modified radical surgery group with significant difference（P〈 0.05）. Conclusion There is no significant difference in general quality of life between the breast conserving operation group and the modified radical surgery treatment group. The body functions and other related factors of breast cancer in the breast conserving patients are better than those of the modified radical surgery patients. But emotional function of the former is worse than that of the latter. In order to improve postoperative QOL of breast cancer patients, pertinent nursing intervention is necessary for improving of emotional function.
