经腹腔镜D2远端胃癌根治术19例

摘　　要：目的探讨经腹腔镜D2远端胃癌根治术手术操作的方法及近期效果。方法对19例进展期胃癌经腹腔镜行D2远端胃癌根治术。结果19例中,除1例中转开腹外,其余18例均成功进行腹腔镜手术。手术平均用时：348（285-449）min。术中平均出血量：194.0（60-310）ml,平均清扫淋巴结19.4（11-43）枚。术后病人平均胃肠功能恢复时间78（36-120）h,下床活动时间79（24-112）h,进食时间68（46-96）h,术后近期生活质量良好。结论经腹腔镜D2远端胃癌根治术安全、可行,与开腹手术的根治效果相当,且有创伤小、术后康复快等优点。Objective To prove the safety, efficacy, and clinical outcomes of laparoscopy - assisted distal gastrectomy and D2 lymph node dissection. Methods A total of 19 patients with gastric cancer were performed distal gastreCtomy. Results Of the 19 patients, 18 were performed laparoscopic-assisted distal surgery successfully and the other one was converted to open surgery. The average operative time was 348 （285 - 449） minutes. The average blood loss was 194 （60 - 310） ml. The average number of the harvested lymph nodes was 19.4 （ 11 -43 ）. The averagc time for gastrointestinal function recovery, for the patients＇ standing up and for the start of oral liquid taking was 78 （36 - 120） , 79 （24 - 112） and 68 （46 -96） hours after operation, respectively. The patients＇ recent recovery and living quality were good. Conclusion Laparoscopy-assisted radical gastrectomy is safe, feasible, and minimally invasive and can achieve the same result of abdominal opening operation.
