100例食管鳞癌Survivin及Bax表达研究

摘　　要：目的研究Survivin、Bax在揭阳地区食管鳞癌的表达及与临床病理情况的关系。方法采用免疫组织化学方法标记100例ESCC标本肿块、癌旁及正常黏膜中SVV、Bax蛋白的表达情况,并对性别、病理分型、分化、分期表达情况作统计比较。结果100例ESCC标本SVV、Bax于肿块与癌旁、正常黏膜位置中阳性率分别为(67%、52%、13%及26%、32%、37%);在蕈伞、溃疡、髓质、缩窄型阳性率为(25%、74.5%、63.6%、58.3%及75%、36.2%、36.4%、33.3%);在高、中、低分化阳性率为(35.3%、51.8%、70.4%及47.1%、37.5%、40.7%);T1T2期及T3T4期阳性率为(30.4%、63.6%及56.5%、32.5%)。结论揭阳地区ESCC的SVV表达在肿块与癌旁、正常黏膜,不同分化情况中明显不同(P<0.05),Bax则无明显统计学意义;SVV、Bax在不同病理分型、分期表达则均有不同(P<0.05),但两者表达情况与性别及淋巴结转移与否无明显统计学意义。结合SVV、Bax表达情况,可辅助判断ESCC的恶性程度及临床预后情况,对本地区食管癌的发生、发展、治疗及预后提供一个较好临床辅助参考指标,使本地区食管癌能较早诊断、治疗。Objective To investigate SVV and Bax expression in esophageal squamous carcinoma in Jieyang area and the clinical pathologic relationship. Methods 100 cases of ESCC preparation were stain with immunohistologic chemistry and compared the expression of SVV and Bax in cancer and cancer surrounding area and studied the sex, pathologic typing, differentiation, staging differences. Results in 100 cases of ESCC the positive expression in cancer, cancer surrounding and normal mucosa were 67%, 52%, 13% and 26%, 32%, 37%.respectively, in verrucous ulcerative, medullary and constrictive type were 25%, 74.5%, 63.6%, 58.3% and 75%, 36.2%, 31.4%, 33.3%, respectively.Positive rate in high, moderate and low differentiation were 35.3 %, 51.8%, 70.4%, and 47.1%, 37.5%, 40.7%, respectively, stage T1 and T2 and T3, T4 positive rate were 30.4%,63.6% and 56%, 35%, 32.5%, respectively. Conclusion The expression of SVV in Jieyang area, ESCC in cancer , cancer surroundings and normal mucosa and different,differentiation had conspicuous difference （P 〈0.05） Bax had no remarkable statistical meaning SVV, Bax in different pathologic typing,staging difference had different expressions （P 〈0.05）,but the difference and the sex and lymphatic metastasis rate had no statistical meaning, combined with SVV and Bax expression can help to decided the malignant degree and clinical prognosis and help to offer better clinical criteria of Jieyang area ESCC, improve better therapy and prognosis and help to have a better early diagnosis and therapy.
