观察急性脑梗死患者在不同头位时的脑血流速度变化

摘　　要：目的研究不同倾斜角度时急性脑梗死患者的脑血流速度变化。方法采用双通道经颅多谱勒超声(TCD)等技术观察24例急性脑梗死患者在不同角度(0°、30°、60°)头高斜位下的心率、血压、脑血流速度的变化。结果在倾斜过程中,双侧脑血流速度均变慢,患侧血流速度减慢幅度明显(P<0.05),每个角度的第1min的血流速度高于第5min的血流速度;血压随着倾斜角度增加而下降(P>0.05),而心率明显加快(P<0.05)。结论在头高斜位下,随着倾斜时间延长急性脑梗死患者脑血流速度减慢,倾斜试验对评价急性脑梗死患者的脑血流变化具有临床价值。Objective To explore the change of cerebral blood flow in acute cerebral infarction patients during tilt table test.Methods Cerebral blood flow in middle cerebral arteries were measured with Transcranial Doppler sonography in twenty-six acute cerebral infarction patients in 0° , 30° and 60° head-up position. Heart rate and blood pressure were also obtained continuously from electricardiograph.Results A downward trend was observed in mean flow velocity with increasing tilt angle in two groups（P〈0.05）,but the changing extent of flow velocity was significant in 60° position in the ischemic hemisphere.The flow velocity of the first minute was greater than that of the 5th minute in every position.A downward trend was observed in blood pressure ,but heart rate quickened significantly with increasing tilt angle.Conclusion Cerebral blood flow becomes slow in acute cerebral infarction patients.It is of value for acute cerebral infarction patients to investigate the variability of cerebral blood flow with tilt table test.
