摘　　要：目的：研究连续静脉静脉血液滤过（CVVH）治疗重症急性胰腺炎（SAP）的疗效。方法：2002～2006年我院86例SAP病人,分为CVVH治疗组42例和HD（间歇性血液透析）组44例,监测病人治疗前后症状、体征及生化指标的变化,行动脉血气分析,APACHEⅡ评分,计算Kt/V值,记录转归情况。结果：①CVVH治疗组血流动力学稳定性好于HD组;②CVVH治疗组Kt/V值高于HD组;③CVVH治疗组治疗后APACHEⅡ评分低于HD组。结论：CVVH治疗SAP病人在血流动力学稳定性、溶质清除率及预后方面优于HD。Objective： To study the influence of continuous venovenous hemofitration（CVVH） on patients with severe acute pancreafitis（SAP）. Methods：86 patients with SAP were divided into CVVH group（42） and HD（44） between 2002 - 2006. Their general conditions, liver and kidney functions, arterial blood gas were observed. Kt/V was calculated after each therapy. APACHE II score was assessed before and after the treatment and the prognosis of each patients was recorded. Results： CWH group had stable hemodynamics than HD group. CWH group had more high Kt/V than HD group. APACHE II score of CVVH group was lower than that in HD group. The survival time of CVVH group was longer than HD group, but the mortality of the two groups was the same. Conclusion： Compared with HD group , CVVH group has more advantage in hemodynamics and solute clearance and prognosis .
